You will be responsible for holding a caseload of service users and for the delivery of a variety of specialist Occupational Therapy assessments and mental health interventions leading on the Occupational Therapy contribution to: assessment, interventions, care planning and evaluation. The overall purpose of the post holder is to ensure that individuals who are referred into the service have their occupational needs assessed and interventions/recommendations are identified to meet these needs and that these are incorporated into an overall formulation of need and appropriate plan of care. You will also be expected to be available for consultation, support and clinical supervision to other health professionals working within the Community Mental Health Pathway Service and other areas of the Trust where identified as appropriate.

Main duties and responsibilities Takeresponsibility, for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy. Administer medication,complying with relevant Trust procedures and NMC Guidance. Assess patient's individual care needs, develop, implement and evaluate programmes of care. Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including; pharmacological and psychologically informed interventions. Complete and contribute, where appropriate to robustrisk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence. Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions. Utiliseagreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process. Participate in the developmentof the service to ensure clinical governance processes are actively implemented at team level. Assist in ensuring that the aims and objectives of the ward/department (as set by the responsible nurse) are fulfilled and to identify factorswhich may inhibit these from being achieved. The post holderwill be required to work within the NMC Code of Practice and professional guidelines. Maintain an up-to-date level of professional competence within the specific environment. Assist in the implementation of evidence-based nursing practice and participate in research and other projects as required.

An opportunity has arisen for a self motivated and experienced nurse to join our band 7 team within the Emergency centre at the Princess Royal hospital Accident and Emergency department.

NHS Shropshire, Telford and Wrekin Integrated Care Board (ICB) is looking for a highly motivated and experienced Senior Clinical Pharmacist to work within the Medicines Management Team covering a fixed-term secondment within the team. They will support the Lead Pharmacist for Medicines Value and Transformation in driving forward the ICB’s Medicines Optimisation Strategy and Medicines Value agenda providing high quality, evidence-based prescribing advice and medicines optimisation support. They will take the lead in developing, implementing and monitoring projects to optimise medicines use, improve patient outcomes and enhance productivity and value for money for the system and wider NHS, enabling long-term financial sustainability.

NHS Integrated Care Boards (ICBs) are responsible for the planning, commissioning and monitoring of health and care services from a range of providers, to support local people.

As an applicant, you must be a registered Pharmacist with the General Pharmaceutical Council (GPhC).
