The Mid-Liaison Team offers a comprehensive range of mental health specialities within one multi-disciplinary team, including liaison psychiatry specialist assessments and support, so that all patients with a suspected or actual mental health problem within an acute hospital setting can be assessed, treated, signposted or referred appropriately regardless of age, presenting complaint, time of presentation or severity of condition.

The service operates 24 hours a day, 7 days a week service delivery based on a rostered shift pattern. Mid-Liaison service is delivered by Essex Partnership University NHS Foundation Trust with the Mid-Liaison Team located within the Broomfield’s Hospital Accident and Emergency (A&E) department.

The post holder will be responsible for ensuring that an integrated service is provided to Patients and ensure that a good inter-disciplinary working environment is maintained. It is expected that the Clinical Lead will formulate, adopted and promote effective collaborative working relationships with specialist lead clinicians in mental health and staff within the acute hospital ensuring that clinical practice development is an inherent part of the service delivery within the Mid-Liaison Team.

The post holder carries responsibility for the assessment and advising on management of patients presenting with mental health difficulties on wards and within the A&E department andwill support and enable specialist expertise in integrated physical and mental health approaches and for intensive support for patients presenting with suspected Dementia or have a diagnosis of Dementia. In addition, there is a requirement to provide support in the active management of those with psychological issues related to frequent attendances to the acute hospital.

Essex County Council is a member of the Southend, Essex, and Thurrock Transforming Care Partnership, consisting of the 7 CCGs and three Local Authorities. As part of these arrangements, it is hosting their All-Age Health Equalities Team, working across this whole footprint, to lead the transformation of the health and social care system, to deliver improved health outcomes for people with learning disabilities and/or autism.

This role has specific responsibility for the administration necessary for the effective delivery of the Southend, Essex, and Thurrock All Age Dynamic Support Register.

Responsibilities:

• Act as the first point of contact for the Dynamic Support Register, Pathway, and its programmes of work, dealing with routine and specialist enquiries to communicate relevant information to stakeholders, referring to others as appropriate.
• Arrange contact or meetings with organisational/individual stakeholders, including families of individuals on the register, to take minutes, collate information and prepare documentation as required.
• Work closely with key teams (such as CAMHS CETR team and ELDP specialist LD Health provider, case managers and social care teams) to ensure up to date information is reflected in the register.
• Responsible (alongside the commissioner) for developing and maintaining a database to securely hold an All-Age dynamic support register with risk stratification and links to agreed pathways.
• Develop and manage systems to ensure safe retrieval, storage and return of any medical notes, social care files or other sensitive personal data, ensuring timely progression to Care (Education) and Treatment Review (C(E)TR) escalating any issues to the LD HE Commissioner for the Dynamic Support Register and Pathway
• Maintain local and nationally required data sets relating to individuals in the community or in hospital requiring a Care (Education) and Treatment Review and the independent personnel required, ensuring a pro-active approach to their delivery is achieved.
• Collate and evaluate data relating to the DSR, Transforming Care and other aspects of the Health Equalities Strategy, to provide accurate and timely information to support local and national reporting deadlines.

Skills, Knowledge and Experience

• Educated to RQF level 3 (A level) or equivalent by experience.
• Evidence of development within role.
• Experience of working in an administrative environment, with a working knowledge of Microsoft Office including Word and Excel and other computerised data systems, with evidence of accurate data entry.
• Awareness of a range of health or social care services and the national initiatives relating to LD and Autism.
• Experience of working with people with learning disability and or autism.
• Able to work on own initiative, organising and prioritising own workload to set deadlines, evidencing problem solving skills.
• Good communication skills, both verbal and written, with the ability to work effectively as part of a team.

We are seeking an inspiring and dynamic pharmacist to join our senior pharmacy management team as the Pharmacist Team Manager for Adult and Elderly Inpatient Mental Health Services in Mid and South Essex. The role will lead on the robust provision of medicines management expertise to adult and elderly inpatient and urgent care services. These posts would suit experienced pharmacy service managers with uncompromising standards who will strive to make a deep footprint on quality of care we provide to our patients. The post is based in Chelmsford, with responsibility for Mid and South Essex Inpatient and Urgent Care Services. As the service leads the posts direct the whole pharmacy inpatient mental health service on a day-to-day basis. It is an extraordinary time for specialist perinatal mental health services across the UK with growing national and local focus on service development and expanding access for new and expectant mums. The national ambition is to improve outcomes for an increasing number women over a 5-year period through investment in resources and broadening of interventions available. The perinatal period includes from conception up until one year following the birth of the baby and is a critical time for prevention and treatment for women at risk of serious mental ill health. Providing a unique opportunity for intervention and change, improving outcomes for mums, their families and ultimately society, in the longer term.
• To work jointly with care coordinators from CMHTs with a small complex and challenging caseload of pregnant women or those with an infant up to one year old.
• Conduct both single handed and joint high quality mental health assessments as required to identify mental health needs for people referred to the Perinatal Mental Health Service.
• Take responsibility for the management of a defined client caseload as identified in consultation with the Team Manager, Clinical Team Leader and team.
• Utilise the Care Programme Approach (CPA) as defined within the Trust Policy and to act as Care Co-ordinator as required.
• To ensure that all Trust Perinatal service users have a perinatal care plan, this is implemented. This may require attending and contributing to pre and post-birth meetings.
• Ensure risks are effectively managed and interventions are effectively prioritised.
• Maintain effective individualised care through the assessment of health and social care needs, planning, implementation and evaluation of care plans co-produced with service users.
• Utilise the Information Management and Technology system of the Trust to maintain effective communication and clear, concise and contemporaneous documentation for individualised client care
• Establish therapeutic relationships with individual clients and to work with fathers, partners and significant others to provide appropriate education training and support.

The pharmacy department at Broomfield Hospital is a professional, forward thinking department which gives all staff the opportunity to develop their full potential. The successful candidate will also be given an area of responsibility within the pharmacy education and training team to support the development of junior members of staff.

You will be based at the Broomfield Hospital site and will be an integral member of the multi-disciplinary team, working alongside a senior clinical technician to provide excellent care to women and children throughout the Broomfield site. You will be professionally supported by the MSE Clinical Lead Pharmacist - W&C.

You will have excellent communication and negotiating skills working closely with patients and multi-professional teams throughout the women and children’s team. You will be able to work under pressure and alone with excellent prioritisation skills.

You will ideally be (or working towards being) a qualified independent prescriber having achieved a minimum of a clinical diploma in pharmacy practice (or having equivalent experience).

A proportion of your working week will be allocated towards supporting the department’s education and training strategy. The team provides support to undergraduate pharmacy students, foundation trainees, pre-registration trainee pharmacy technicians, and staff undertaking a range of postgraduate qualifications.
