• Conduct comprehensive assessments of individuals with learning disabilities in the context of mental health, this may include environmental and personalised sensory assessments

Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas. Provide person-centred medicines optimisation and quality improvement, whilst contributing to the Quality and Outcomes Framework (QOF), Impact and Investment Fund (IIF) and Enhanced Services. Develop new pathways when necessary and work with existing ones, such as AccuRx, to improve efficiency, safety, and quality of care. Work with the nursing team to create a coherent chronic disease review system by aligning recalls and medication reviews and identifying and eliminating duplication of work to improve efficiency and patient care. Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care. Have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare, and help manage general practice workload.
