• The outpatient MH RN is responsible for providing patient-centered and recovery-oriented care and maintaining self-awareness while caring for patients living with MH conditions.

As independently licensed health professionals, CRNAs are responsible and accountable for their practice. • The Care Coordination & Integrated Case Management (CC\ICM) Social Work Program Coordinator, in collaboration with a Nursing CC\ICM counterpart, is responsible for implementation, coordination, and management of all administrative and clinical aspects of the CC& ICM framework.
• The Social Work Program Coordinator for CC\ICM will implement standardized tools designed to integrate care to create a seamless continuum experience for the Veteran.
• The CC\ICM framework is a specialized, collaborative practice among multiple healthcare professions which provides structure and standards to support inter-professional collaboration throughout the continuum of care and optimal utilization of healthcare resources. CC\ICM serves a highly complex Veteran population, whose problems require a higher level of care coordination.
• Identified Veterans involved in CC\ICM are ensured to receive the most appropriate level of care coordination support, based on their prominent need.
• The aim of CC \ICM is to provide safe, efficient, and cost-effective interventions integrated throughout the continuum of healthcare; to improve access to care, reduce duplication and fragmentation of services, ensure adherence to professional standards of practice, and support staff in their work.
• Administrative responsibility for the clinical programming and daily operations of the Montana VA Healthcare System (MTVAHCS) CC\ICM Practice.
• Developments and implements policies and procedures specific to MTV AHCS and ensures compliance with local facility, VISN, the Associate Director of Patient Care Services (ADPCS), Social Services, VA Central Office (VACO) directives and handbooks, as well as Joint Commission, and other accrediting bodies and regulatory standards as needed.
• Ensures full collaboration with nursing counterpart, for all planning, development, implementation, monitoring, and evaluation of the CC\ICM operations.

• Collects specimens for blood glucose analysis using the blood glucose monitoring instrument. Enters and verifies completed test results, and reports results to appropriate individuals. Calibrates, cleans, and maintains the instrument.
• Completes appropriate laboratory documents to accompany blood samples to the laboratory.
• Transports patients and specimens to required destinations.
• Prepares and assists with examinations, diagnostic, and therapeutic tests according to written procedures and policies.
• Performs and records a variety of advanced specialized treatments that include nasal, nasopharyngeal and/or tracheotomy suctioning, inserting urethral and suprapubic catheters, institutes and maintains bladder irrigations with three-way urethral catheters. Utilizes aseptic techniques when applying compresses or complicated dressings that may include irrigations and packing, and empties Hemovac and Jackson-Pratt drains.
