Data Quality The post holder will: Analyse and interpret data and assist with planning and monitoring work undertaken in relation to Directed, National and Local Enhanced Services, the Quality & Outcomes Framework and the PCN Investment and Impact Fund, taking appropriate action to ensure that all targets are achieved, and accurate claims are returned. Run and develop searches, creating audits and reports on EMIS Reviewing and updating clinical templates ensuring they relate to current practice directives. Guide the practice to reach QOF, IIF (and any other) targets. To manage data quality issues, providing advice and guidance to all practice staff in the use of the clinical system. To set targets and monitoring standards for data entry and data collection undertaken by the administration team. Production of Performance and Quality Information The post holder will: To be aware of and promote national, local and practice quality standards for chronic disease management, to ensure that the quality of care is maintained at a high standard using NICE Guidance and the existing GP Contract as a guide. To oversee all the processes required to produce the clinical achievement standards for a. QOF (Quality Outcomes Framework) b. Enhanced Services (both local and national) c. Prescribing audits d. CQC (Care Quality Commission) e. PCN DES and IIF (Investment & Impact Fund) f. Any other initiatives which are asked of us. To be the lead for clinical audit within the practice and assist staff in clinical searches together with the collation and interpretation of patient data To provide support to the clinical teams in correctly identifying and targeting patients for assessment and treatment. To ensure staff are aware of the importance of maintaining disease registers and assist in the validation process. To provide advice and support for the clinical teams and to produce regular reports to assist the progress of these groups in achieving their targets using appropriate presentation techniques that can be understood by staff and additional commentary to highlight issues, trends and areas of interest. To assist in the production of information for clinical audit as requested by the clinical teams and make recommendations to complete the audit cycle. Routinely monitor and assess practice performance against patient access and demand management targets, including national performance dashboards To validate information provided by the Shropshire Telford & Wrekin CCG Data Quality and Maintenance of Patient Records The post holder will: Oversee all methods of data entry and ensure templates and protocols are reviewed and updated as required. To set up systems for validation of patient information, performing regular checks and quality audits. Information Governance The post holder will: Ensure that medical records are held securely within the practice and data is held in line with the NHS Codes of Practice for Records Management, the RCGP Good Practice Guidelines for electronic records, GDPR, Caldicott Principals and Freedom of Information Act. To ensure that policies relating to data security, accuracy, protection and sharing are regularly reviewed, updated and communicated to all staff. Management of Clinical System The post holder will: Ensure the clinical integrity of the system liaising with the system supplier and STWCCG to implement their guidance. To provide support advice and training for current and new practice staff in the use of IT software. Practice Lead for IT Hardware, Software. The post holder will: With the support of the Operational Manager, to lead and oversee the management of any changes in our clinical system. This includes liaising with STWCCG/MLSCU over appropriate hardware and software as well as ensuring appropriate processes are in place for all staff to be trained. Liaise with STWCCG/MLSCU to ensure accurate asset management of all IT equipment and service continuity plans are in place. To liaise with MLSCU support system suppliers to resolve other hardware and software issues. Leading and supporting the Team Leaders to ensure the Practice has a strong social media presence and that campaigns, messages and practice updates are communicated effectively using the website, facebook, twitter and waiting room screens. Communications The post holder will: Foster good relationships with patients, practice staff and visiting professionals. Liaise closely with other members of the Primary Care Network Team. Ensure communication systems are running smoothly, doctors and reception staff are kept fully informed of changes in procedures to meet targets. Collaborative Working All staff are to recognise the significance of collaborative working. Teamwork is essential in multidisciplinary environments. Effective communication is essential, and all staff must ensure they communicate in a manner which enables the sharing of information in an appropriate manner. Health & Safety The post-holder will: Assist in promoting and maintaining their own and others health, safety and security as defined in the Practice Health & Safety Policy, to include: Using personal security systems within the workplace according to Practice guidelines Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks.
