Adventist Healthcare seeks to hire an experienced Assistant Manager RN or Social Worker for our Care Navigation Behavioral Health Department who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing.
As an Assistant Manager RN or SW, you will:
•Support departmental and organizational financial goals of minimizing potentially avoidable utilization and length of stay as well as reducing readmission rate.
•Lead team to ensure that delays to care are minimized and that effective discharge plans are developed to navigate patients to the most appropriate level of care in a timely manner (for inpatient).
•Lead team to collaborate with the acute care teams, post-acute care teams, and physician practices to ensure patients are navigated to the most appropriate level of care in outpatient setting (for outpatient).
•Ensure active participation in Interdisciplinary Rounds and other patient care conference
•Lead delivery of care provided by Care Navigation team to ensure that patients’ needs are addressed, and that documentation meets standards.
•Implement the departmental strategic plan.
•Collaborate with Utilization Review leadership to understand trends of discharge barriers and denials.
•Facilitates the development of systems, processes and PI that drives optimal performance for the department.
•Oversee and evaluate the performance of the Care Navigation team.
RN Qualifications include:
•Degree in nursing (Associate, Diploma or Baccalaureate)
•Baccalaureate degree in nursing preferred
•Current Maryland State nursing license
•Certification in department-specific area within the first year of the position
•Two years’ work experience preferred in department-specific area
•One year supervisory experience preferred
•Active American Heart Association Basic Life Support (BLS) certification required
SW Qualifications include:
•Master of Social Work required
•3 – 5 years’ experience in patient navigation, discharge planning, or care coordination
•At least one year of prior leadership experience preferred
•Active Maryland license as LCSW-C or LCSW with plans to achieve LCSW-C in 1 year
•Active American Heart Association Basic Life Support (BLS) certification required
Work Schedule:
Full-time, Monday- Friday.
