POSITION SUMMARY
This position performs a variety of support functions, under the supervision of clinical pharmacists, to contribute to the overall improvement of healthcare quality as well as efficient use of resources. Primary responsibilities include processing prescription renewal requests received electronically in the electronic health record, via fax, or phone call; data management and pre-screening to support pharmacist disease state management services, including patient outreach to enroll patients in the service; and patient outreach to assist with closing quality measure care gaps. Other support functions include data gathering and entry, electronic document filing, electronic faxing, and use of the EHR. Additional responsibilities may include outreach to physician offices.

Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.

Must have two or more years of ambulatory pharmacy experience (retail/community or managed care) or demonstrate knowledge of ambulatory pharmacy. Lead and participates in institution wide improvement activities through sharing lessons learned and best practices to facilitates continuous improvements.

6. Coordinates recruitment and interview events and logistics. Ensure all annual credentialing and other hospital and Department requirements are met for each trainee within the assigned program.

7. Collaborate with the Program Director, APD, and/or Program Chief(s) to design and implement yearly rotation and vacation schedules for house staff. Work performed is reportable to the Graduate Medical Education Leaderships. Serves on at least one GME subcommittee annually.

8. Maintains accurate records on all current and former house staff necessary to verify successful completion of residency program using the Residency Management Software Prepares all cyclical reports for residents, staff and attending physicians. Maintains discretion relative to confidential and sensitive residency/fellowship issues.

9. Internal contacts include physicians, administration, and employees. External customers include various medical education accreditation agencies and federal, state and local regulatory agencies for medical education.

MINIMUM QUALIFICATIONS

Must possess knowledge of medical education coordination as normally obtained through the completion of a bachelor’s degree in business, human relations or related field.

Must have C-TAGME or acquire the certification within one year. Must possess or obtain active Notary Public commission within 90 days of hire.

Must possess an understanding of graduate medical education as normally demonstrated with five years of experience in graduate medical education. POSITION SUMMARY
This position assists technologists and/or nurses in department by performing non-technical, patient and procedure-related duties in accordance with department/facility policies and procedures. This position is also responsible for a variety of clerical and administrative support functions and customer service for an assigned patient care area which promote an organized, efficient delivery of care to patients.

CORE FUNCTIONS
1. Performs a variety of tasks and functions associated with the department’s daily operations, including but not limited to, greeting patients, medical staff and the public, answering incoming telephone calls, scheduling appointments, entering orders and providing general information to customers. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution. Refers complex inquiries to the appropriate department or staff. Provides ongoing clerical and administrative support.

2. Enters patient information into the computer, scans documents into PACS. Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient’s record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. May be responsible for QC/validations. Assists in facilitating requests for patient medical records.

3. Provides support in the acquisition and distribution of digital patient records. Digitizes, formats, and uploads film-based or digital images into PACS. Maintains accurate filling system as required based on established department protocols and procedures.

4. Fulfills requests for images and reports ensuring availability in system, hard copy (film), and/or CD as required and follows proper protocols for releasing confidential patient information.

5. Receives patient in applicable systems, confirms patient demographics and insurance information and performs insurance verification when required. May be required to calculate and collect copays, deductibles, and self-pay. Totals charges on patient’s super-bill at check-out. Enters pending discharges into the system. Coordinates patient appointments for procedures and treatments that may be required prior to timely discharge. Processes patient discharges. Coordinates transportation to care centers and residential homes as requested by patients or physicians.

6. Prints physician schedules, super-bills, labels and other applicable forms in preparation for next business day. Pulls and files patient charts and prepares new patient charts as needed. Prints and files transcription notes. Reconciles charge tickets to each day's schedule, identifies incomplete tickets, missing charge codes or missing diagnosis and notifies clinical staff. Forwards completed tickets and/or billing package to the business office or billing company.

7. Assists technologists and nurses in pre and post procedure room preparation by obtaining necessary supplies and assisting patients as necessary. Ensures procedure rooms are stocked appropriately. Understand sterile procedure and assists with setting up sterile trays, where applicable. Adheres to radiation safety policies. Escorts patients back to the patient exam rooms, takes vitals and patient history; may assist with patient treatment to include peripheral IV insertion, IV discontinuation and Point of Care Testing (POCT) if required by the assigned facility (completion of Banner’s peripheral IV insertion course and validation of skill must be completed prior to any peripheral IV insertion and discontinuation performed. Completion of Banner’s POCT education and validation of skill must be completed prior to any Point of Care Testing being performed).

8. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery.

9. This position assists in the operations of the medical imaging department under direct supervision with technologists and nurses in department. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Must navigate a variety of separate computer programs at any given time and handle multiple demands on time. Internal customers: All levels of medical imaging and nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: regulatory and health agencies, non-clinic providers and facilities, vendors, patients and family members.

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge.

Certification for BLS is required.

Must be able to exercise good judgment, effectively communicate, and use discretion with patients, staff and the general public. Working knowledge of medical terminology and computer literacy is required. The position requires knowledge of commonly used office software and keyboarding skills. Good organizational skills are essential in prioritizing and coordinating various responsibilities simultaneously. POSITION SUMMARY
This position provides leadership in specialty area of clinical nutrition, establishes practice standards and serves as a mentor to educate colleagues in the specialty nutrition area. This position manages a complex caseload of patients by providing comprehensive nutritional assessments and nutrition-focused physical exams, developing and managing all aspects of appropriate nutrition care plans, and monitoring outcomes of plans. Provides patient and family nutrition counseling and education. Develops Nutrition Standards of Practice and Education material in collaboration with peers. Serves as a member of the interdisciplinary Health Care team providing input and establishing patient care policies, programs, quality improvement activities and provides in-service education in areas of expertise.

CORE FUNCTIONS
1. Provides comprehensive nutrition assessment of patients' nutritional needs, to include nutrition-focused physical exams as appropriate, with at least 50% of patient case load in designated specialty area. Obtains timely and appropriate data and analyzes/interprets data based on evidence-based standards and established practice guidelines.

2. Demonstrates and maintains competency in enteral and parenteral clinical practice protocols and oral nutrition management standard of practice as demonstrated by successful completion of initial competency exams and ongoing evaluation.

3. Writes orders to initiate and manage the nutrition care and of all assigned patients including those receiving nutrition support (parenteral and enteral nutrition) per protocols.

4. Evaluates effectiveness of the care plan and communicates effectively to the other members of the healthcare team to provide optimal nutrition outcomes. Provides pertinent information in the medical records, patient plans of care and/or clinical pathways while meeting requirements of regulatory agencies (i.e. Joint Commission, CMS, State and Local Agencies).

5. Provides nutrition education to patients, families, staff and other health care team professionals through the development of effective teaching materials and in-service programs; Evaluates patient and or family response to education.

6. Provides leadership in area of practice, establishing proper patient care procedures and acts as liaison with nursing areas. Establishes and monitors policies and procedures in area of expertise and communicates areas of concern that impact patient care in this area. Assists with program marketing through the development of effective work relationships with internal and external customers to enhance the use of services provided by clinical program. Serves as mentor to new staff and interns where assigned.

7. Participates in and identifies continuous quality improvement activities in clinical areas to ensure optimal nutrition care outcomes are achieved; Participates in clinical nutrition Continuous Quality Improvement (CQI) activities and completes one outcome study per year to improve on quality of nutrition service or to improve nutrition outcomes. Provides additional coordination of projects and programs.

8. Demonstrates proficiency in area of practice; Responsible for developing training and educational materials to assist colleagues in developing skills in area of specialty and mentors staff members in area of specialty.

9. This position interacts with patients, families, health care providers and nutrition and dietary staff in support of providing high quality nutrition assessment, care planning, delivery and measurement of outcomes. MINIMUM QUALIFICATIONS

Must possess the knowledge of nutrition and dietetics demonstrated through the completion of a Bachelor's Degree in Nutrition and/or Dietetics. Must be a Registered Dietitian based on academic and professional requirements as defined by by the Commission on Dietetic Registration (CDR) of The Academy of Nutrition and Dietetics. Effective January 1, 2024 CDR requires a minimum of Master's degree to be eligible to take the registration examination. Must hold current registration through the Commission on Dietetic Registration of The Academy of Nutrition and Dietetics and be able to maintain ongoing registration while providing verification of registration annually. Maintains current licensure as required by State law.

As an MIA you will help organize work flow, assist the Technologist with transferring patients to exam table, cleaning workspaces, stocking rooms, transporting patients, if necessary, all while having face to face contact with patients. May assist with patient treatment to include peripheral IV insertion, IV discontinuation and Point of Care Testing (POCT) if required by facility.

POSITION SUMMARY
This position assists technologists and/or nurses in department by performing non-technical, patient and procedure-related duties in accordance with department/facility policies and procedures. This position is also responsible for a variety of clerical and administrative support functions and customer service for an assigned patient care area which promote an organized, efficient delivery of care to patients.

CORE FUNCTIONS
1. Performs a variety of tasks and functions associated with the department’s daily operations, including but not limited to, greeting patients, medical staff and the public, answering incoming telephone calls, scheduling appointments, entering orders and providing general information to customers. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution. Refers complex inquiries to the appropriate department or staff. Provides ongoing clerical and administrative support.

2. Enters patient information into the computer, scans documents into PACS. Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient’s record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. May be responsible for QC/validations. Assists in facilitating requests for patient medical records.

3. Provides support in the acquisition and distribution of digital patient records. Digitizes, formats, and uploads film-based or digital images into PACS. Maintains accurate filling system as required based on established department protocols and procedures.

4. Fulfills requests for images and reports ensuring availability in system, hard copy (film), and/or CD as required and follows proper protocols for releasing confidential patient information.

5. Receives patient in applicable systems, confirms patient demographics and insurance information and performs insurance verification when required. May be required to calculate and collect copays, deductibles, and self-pay. Totals charges on patient’s super-bill at check-out. Enters pending discharges into the system. Coordinates patient appointments for procedures and treatments that may be required prior to timely discharge. Processes patient discharges. Coordinates transportation to care centers and residential homes as requested by patients or physicians.

6. Prints physician schedules, super-bills, labels and other applicable forms in preparation for next business day. Pulls and files patient charts and prepares new patient charts as needed. Prints and files transcription notes. Reconciles charge tickets to each day's schedule, identifies incomplete tickets, missing charge codes or missing diagnosis and notifies clinical staff. Forwards completed tickets and/or billing package to the business office or billing company.

7. Assists technologists and nurses in pre and post procedure room preparation by obtaining necessary supplies and assisting patients as necessary. Ensures procedure rooms are stocked appropriately. Understand sterile procedure and assists with setting up sterile trays, where applicable. Adheres to radiation safety policies. Escorts patients back to the patient exam rooms, takes vitals and patient history; may assist with patient treatment to include peripheral IV insertion, IV discontinuation and Point of Care Testing (POCT) if required by the assigned facility (completion of Banner’s peripheral IV insertion course and validation of skill must be completed prior to any peripheral IV insertion and discontinuation performed. Completion of Banner’s POCT education and validation of skill must be completed prior to any Point of Care Testing being performed).

8. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery.

9. This position assists in the operations of the medical imaging department under direct supervision with technologists and nurses in department. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Must navigate a variety of separate computer programs at any given time and handle multiple demands on time. Internal customers: All levels of medical imaging and nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: regulatory and health agencies, non-clinic providers and facilities, vendors, patients and family members.

POSITION SUMMARY

The Cloud platform team is responsible for defining, building, and migrating to Banner’s future state cloud platform while ensuring proper support and enablement of existing cloud assets.  The cloud platform teams mission is to enable better care and experiences through modern cloud-based assets.  We have ownership and responsibility for building a secure, performant, economically viable cloud platform to ensure business velocity.

In this role you will provide day to day technical direction to engineers on the team to ensure adherence to long term strategy.  This director will also own the overall technical delivery and day to day operations & engineering for the future state cloud platform.  This includes building of new capabilities as well as operational support in adherence with Banner ITSM standards. This position will be Hybrid in Phoenix, AZ.

POSITION SUMMARY

This position is the managerial professional who participates in the development of the strategic direction and objectives for information technology solutions. Evaluates and translates company plan and goals, forming department annual operating and capital budgets. Interacts with management across diverse areas and multiple states participating in the evaluation of company plan and initiatives to develop actionable projects and activities in support of the company’s infostructure and infrastructure.

CORE FUNCTIONS

1. Hires, trains, conducts performance evaluations, and supervises the workflow for designated staff. This includes initiating promotions, transfers and disciplinary actions. This includes establishing priorities, workloads, controls and work procedures, as well as determines resources needed.

2. Participates and may lead task forces to plan, implement, and coordinate facility activities to maximize service quality, effectiveness and efficiency.

3. Evaluates and participates in the development of the company strategic direction and plan and determines the high level design, direction, and coordination of information technology projects, and solutions. Ensures that department and company standards are implemented and consistently adhered to, ensuring current and future reliability and functionality.

4. Develops and oversees the department budget in conjunction with corporate goals and objectives. This position is accountable for meeting annual budgetary goals. Formulates multi-million dollar capital and operating budgets in support company’s strategic goals and mission. Assembles feasibility studies, request for information/proposal (RFI/RFP) and return on investment (ROI) calculations in defense of these budgets. Negotiates large scale contracts with vendors and monitors expenditures associated budgets.

5. Provides expertise and is the point person on behalf of information technology to JCAHO, Medicare, auditing firms and other regulatory agencies. Assures that departmental staff is trained and adheres to all local, state and federal regulations.

6. This position has company wide responsibility for a wide range of information technology projects, and hardware and software solutions. Work is performed in a fast paced multi-tasked environment where there may be conflicting priorities or tight deadlines. The incumbent has critical decision-making authority for projects, designs and solutions and works at peer level with management across diverse areas and multiple states. The incumbent develops content and participates in meetings, presentations, visioning sessions, strategic and planning sessions, implementation activities, and supports functions system wide. Maintains and applies a broad expert knowledge of company policies, procedures, and legal and regulatory requirements.

Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. MINIMUM QUALIFICATIONS

Must possess strong knowledge as normally obtained through the completion of a bachelor’s degree in related field plus progressive experience in a healthcare environment, or equivalent combination of relevant education, technical, business and healthcare experience typically achieved in 10 years. Must demonstrate superb knowledge of information technology and healthcare. Needs considerable proven leadership experience in large scale project planning, reporting, and budgeting. Successful candidate will have skills and experience to recruit, mentor, motivate, evaluate, and retain an effective staff.

Requires exceptional communication, presentation, and negotiation skills to engage technical and non-technical audiences. Requires leadership skills with ability to lead, communicate, and interact across facilities and at various levels. As is typical in this industry, variable shifts and hours and carrying/responding to a pager may be required.

Works as a member of the system/region materials leadership team to develop and standardize efficient and effective practices. May be responsible for ensuring that staff is fully trained and certified in forklift equipment operation as well as fully educated and held accountable for all safety procedures.

6. Maintains, updates, researches, and tracks shipping/receiving records for departments, Systems Materials Management, and Accounts Payable. Maintains department records relating to personnel matters, in-service education, accounts payable, supplies received, equipment maintenance, and other related areas.

7. Departmental/facility responsibility. Incumbent assists all departments with ordering, shipping and, receiving functions. Works under limited supervision using independent judgment to achieve goals. Internal customers are other staff, nurses, physicians, patients, visitors, supervisors, managers and executives. External customers are patients, visitors, vendors, other company facilities, and non-company medical facilities.

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge.

Must posses a valid driver’s license and be eligible for coverage under the company auto insurance policy.

Requires the skills, knowledge and experience acquired over three or more years of computerized inventory management and shipping and receiving functions, plus extensive knowledge of healthcare supplies and handling processes

Requires sound math and business skills, intermediate abilities in word processing, spreadsheet and/or database software use, and the ability to research and compile statistical reports. May be required to be forklift certified within 30 days in certain facilities. The Transitional Care Coordinator will work alongside our clinic teams and support our oncology patients with referrals, cardiac clearances, record requests for imaging and transfers. POSITION SUMMARY
This position supports the smooth, timely, and coordinated client transition from acute care to alternative levels of care including but not limited to post-acute settings, community services, or home with post-acute service support, as directed by the care coordination team. This position performs follow-up tasks and coordinates the logistics for a patient’s discharge services identified in the inpatient discharge care plan for management of Banner patients across the healthcare continuum.

CORE FUNCTIONS
1. Works to coordinate the patient’s transition into or out of a care setting and obtains appropriate services and benefits as directed by the care coordination team. This may include faxing information, entering referrals or tasking other departments or consultants, arranging authorization and transportation, arranging durable medical equipment (DME), coordinating home health care, confirming arrangements, making physician or outpatient appointments, obtaining test results, and other patient related duties as designated.

2. Coordinates and manages the logistics of discharge planning for individual patients and works to coordinate the patient’s transition into or out of a care setting and obtains appropriate services and benefits as directed by the care coordination team. This may include faxing information, entering referrals or tasking other departments or consultants, arranging authorization and transportation, arranging durable medical equipment (DME), coordinating home health care, confirming arrangements, making physician or outpatient appointments, obtaining test results, and other patient related duties as designated. Keeps other members of the care team informed of barriers or challenges which might delay the patient’s discharge and works collaboratively with the care team to resolve such challenges.

3. Documents all interventions in the patient medical record both timely and accurately including all elements of the discharge plan. Performs transfer of accurate, pertinent patient information between all appropriate entities of the acute and post-acute care continuum relative to the anticipated discharge/transfer of the patient.

4. Works collaboratively with team members; promotes collaborative relationships with vendors, community and referral resources.

5. May perform tasks such as securing community resources/information or other tasks.

6. Works under general supervision. Confers with supervisor on any unusual situations. Internal customers: Post-acute services team members and all levels of nursing management and staff, medical staff, and all other members of assigned facility interdisciplinary health care team. External customers: home health agencies, nursing homes, insurance providers, group homes, assisted living facilities, hospice, long-term acute care hospitals, inpatient rehabilitation facilities, volunteer agencies, county/governmental agencies and medical supply companies and others as required.

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge.

Certification for BLS is required for acute-care settings where direct patient care is provided.

The position requires a proficiency level typically achieved with one year of experience in healthcare as a Nursing Asst, Medical Asst, Health Unit Coordinator, Patient Care Tech, etc. Must demonstrate effective communication and customer service skills, human relation skills and time management skills with flexibility in responding to multiple demands. Must be able to work flexible hours and work after hours/weekends on rotation.
