East Sussex Healthcare Trust (ESHT) is seeking a dynamic and experienced Social Care Practice Education Lead to join our dedicated team. This pivotal role will operate across both health and social care organisations, focusing on the implementation of a trusted assessor model. The primary aim is to enhance the efficient flow of patients between the Urgent Community Response Team at ESHT and ASC teams, ensuring seamless transitions and optimal patient care.

The urgent community response team is based across two sites, Eastbourne District General Hospital, Eastbourne and Conquest Hospital, Hastings. This post will involve working across both sites..

Key Responsibilities:
• Trusted Assessor Model Implementation:Lead the introduction and integration of a trusted assessor model to streamline patient flow and improve care efficiency.
• Collaboration Across Teams:Work closely with the Urgent Community Response Team and ASC teams to support hospital discharge, admission avoidance pathways, Homefirst, and virtual ward pathways.
• Multi-Disciplinary Coordination:Engage with a large multi-disciplinary team (MDT) comprising nursing, allied health professionals, administration, and social care staff who deliver urgent, short-term support during crises.
• Change Management:Drive change initiatives and ensure effective implementation of new processes across health and social care organisations.
• Patient Care Continuity:Ensure service users receive appropriate care following initial short-term intervention, whether they return to independent living or require longer-term support.
• Educational Support:Develop and implement education and training programs for staff across health and social care settings to ensure understanding and effective utilisation of the trusted assessor model
Candidate Profile:
• Experience:Proven experience in either health or social care settings.
• Leadership Skills:Strong leadership abilities with a clear focus on managing change effectively.
• Communication Skills:Excellent verbal and written communication skills.
• Project Management:Experience in project management or relevant project work is highly desirable.
• Team Collaboration:Ability to work collaboratively within a multi-disciplinary team environment.
• Educational Expertise:Experience in developing and delivering educational programs to support staff development and improve service delivery.
