Our aim is to work together in partnership to join up health and care services, improving our populations health and wellbeing and reducing health inequalities. One such example of this collaborative work is the launch of our HCA Academy, an ICS initiative between Essex Partnership University NHS Foundation Trust (EPUT) and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming Health Care assistant (HCA) workforce. The academy models a one workforce cross system approach to the onboarding, induction, education and experience of Healthcare Assistants. To support this one workforce model the successful candidate for this post will be required to attend our5 day (Monday to Friday) face to face Induction Programme at our MSE ICS Education Hub at Brentwood Community Hospital. Your start date will therefore fall on one of two of the HCA Academy induction dates across the month, which will be discussed and agreed during the recruitment process. Our HCA Academy Induction programme has been designed to train and support the successful candidate through the early stages of their employment at EPUT and will provide opportunities to enhance skills, knowledge and experience. Further information will be provided post interview. This will be followed by an additional 5 day (Monday to Friday) EPUT corporate / mandatory training welcome. (EPUT Welcome will be varied between face to face and virtual delivery).

As a Salaried GP, the successful candidate will manage their own caseload. Dealing with a wide variety of health needs in a primary care setting, they will work individually and as part of a team to ensure a high standard of care for all registered patients is met, as they carry out the associated clinical and administrative duties.

Job responsibilities Key Responsibilities: Clinical practice Independently Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis Clinically examine and assess patient needs and plan clinical care accordingly Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate) Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice Work with patients in order to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care Assess, identify and refer patients presenting with mental health needs in accordance with guidelines.

Duties and Responsibilities: The duties and responsibilities to be undertaken by members of the Practice administration team may include any or all of the items in the following list. Support patients to set their own goals and be confident in their approach to self-management. Evidence, Research, and Development Develop relationships and a collaborative working approach across the PCN supporting the integration of pathways in primary care. Work as part of an MDT in a patient-facing role, using your expert knowledge of MSK issues, to create stronger links for wider MSK services through clinical leadership, teaching and evaluation skills. Support research and audit programmes to evaluate and improve the effectiveness of our FCP service. Streamline the pathway of care by providing a responsive service so that patients receive timely access to care. Leadership Lead, manage and contribute to service delivery. Implement all aspects of effective clinical governance for own practice, including undertaking regular audit and evaluation, supervision and training. Communicate effectively and appropriately, with patients and carers, complex and sensitive information regarding diagnosis, pathology, prognosis and treatment choices supporting personalised care. Facilitating learning Provide feedback and support to other PCN clinical team members on appropriate referral criteria. Critically reflect on your own practice, and continue to develop your skills.
