We are a teaching Trust, teaching undergraduates from Birmingham Medical Schools, St Georges Grenada, and are also part of the Black Country training scheme with core and higher specialist trainees from the West Midlands School of Psychiatry. We have a good record of encouraging and supporting specialty doctors if their aim is to achieve postgraduate exams and go on to Higher Specialist Training. There are good opportunities for applicants interested in education and research. Specialty Doctors will cover on average 1:11 out of hours second on-call rota.

To extract blood samples from patients/donors in a manner that makes the blood sample suitable/fit for analytical, therapeutic or diagnostic testing and purpose. Working in hospital wards, clinics, community-based clinics within other organisations and NHS Trusts and also in patients homes. There is a requirement to work weekends, bank holidays and cover service delivery times between 08.00 and 20.00 as the roster dictates. The jobholder will be undertaking a training programme within the Leeds Teaching Hospital NHS Trust (LTHT). The training will be delivered through formal education either in-house and or via external providers, and clinical placements. They will be involved in providing direct and indirect care for patients in support of and supervised by a Phlebotomist and where identified as appropriate by the Phlebotomy Supervisor. It is expected that the apprentice will commit to self-directed study and successfully complete the training programme within the contract period. There will be commitment to a minimum 2 general cardiology per week as well as a contribution to the general cardiology Specialty Registrar rota (full shift). The successful applicant will also be expected to support inpatient care under the supervision of the consultant team. An expanding research and clinical trials programme is in place in which the successful candidate will be expected to participate fully, with a protected session for research and audit in the timetable. A tailored training programme can be negotiated around this service provision. LEVELS OF RESPONSIBILITY This will depend upon prior experience and can be discussed. It is expected that the levels of responsibility will be that of ST6 or above, and the successful candidate will receive supervision from a named consultant with responsibility to develop the individuals training e-portfolio or equivalent. TEACHING AND RESEARCH/STUDY AND TRAINING Whilst the advertised post is a Trust Doctor, it is expected that the successful applicant will be able to access opportunities to attend weekly teaching and monthly educational departmental meetings. INFORMATION ABOUT THE DEPARTMENT This is a 12-month fixed-term post for a Senior Clinical Fellow working within the Upper Gastrointestinal Surgery Department based at St Jamess University Hospital. The unit is a tertiary centre for referrals for oesophago-gastric cancer surgery as well as providing upper gastrointestinal and bariatric surgery services to Leeds. This post will be suitable for a trainee who is taking or who has just taken the UK CCST exams or international equivalent and will be for 12 months. Previous experience in Upper GI surgery is essential. Given the tertiary nature of much of the unit's work, a post-CCT or an experienced senior surgical trainee are likely to get the most benefit from the Fellowship. The unit comprises six surgeons. All six surgeons undertake oesophago-gastric cancer resections, bariatric surgery, anti-reflux surgery and general surgery (e.g laparoscopic cholecystectomy and hernia surgery). There is training in laparoscopic and open oesophagectomy and gastrectomy and a robotic program started in 2022. Cancer training is supplemented by broad training in Upper Gastrointestinal Surgery including bariatrics, anti-reflux and hiatal surgery. There is a JAG accredited endoscopy unit and training can be provided to the candidate depending on their level of experience and interests. MEDICAL STAFF IN THE DEPARTMENT The post-holder along with 2 other Speciality registrars will have access to 4.5 full day operating sessions per week in main theatre and day surgery lists, specialist multidisciplinary meetings, outpatient clinics and endoscopy lists. This post will be allocated to the Department of Upper Gastrointestinal and Minimally Invasive Surgery led by 5 Consultants. The unit has expertise in the management of oesophagogastric cancer, complex benign upper GI disease, minimally invasive surgery and obesity surgery.

To provide a comprehensive and high quality medical secretarial service to support clinical teams (Consultants, their medical teams e.g. junior medical staff or specialist nursing staff as appropriate) in delivering high quality care to the patients of Leeds and beyond. This role will additionally make a key contribution in improving the patient experience by supporting Directorate teams with the movement of patients through their 18-week pathway or other national waiting time standards as appropriate. SPECIAL WORKING CONDITIONS i) PHYSICAL EFFORT: The role requires high levels of accuracy in producing clinical correspondence that meets a high-quality standard as part of the provision of an effective secretarial service. There will be long and frequent periods where the post holder will be in front of a VDU either typing or proof reading. The post holder may be required to lift small loads and handle stock and stationery e.g. paper in and out of their Department. ii) MENTAL EFFORT: Prolonged periods of concentration are required for transcribing or proofreading medical correspondence and using patient related information. This may frequently be interrupted with phone calls or via face-to-face contact with other members of the team e.g. Consultants. iii) EMOTIONAL EFFORT: The post holder will be handling sensitive patient-related information daily and as such may be regularly (daily) subjected to difficult and emotional situations such as serious illness, bereavement or anxious family members. iv) WORKING CONDITIONS: The post will require use of a VDU and keyboard on a daily basis.

This post is a full time (10 PA) Locum post, fixed term for a period of 6 months. The appointee will work at St Jamess University Teaching Hospital, where there will be inpatient and outpatient responsibilities. There is a commitment to Respiratory Medicine 1:12 on call and Acute Medicine 1:109 on call. The applicant should be committed to teaching and research. 15 whole time equivalent consultant respiratory physicians provide a cross-city inpatient and outpatient service. Current sub-specialty interests among the physicians include lung cancer, pleural disease, interstitial lung disease, difficult asthma, tuberculosis, sleep disordered breathing & non-invasive ventilation, cystic fibrosis, bronchiectasis, PCD, COPD and alpha-1-antitrypsin deficiency The Leeds TB Service We offer an integrated service including diagnosis and treatment of active Tuberculosis and Latent Tuberculosis Infection, and screening for people who have been in contact with tuberculosis. In addition, we manage patients with pulmonary non tuberculous mycobacterial disease. Respiratory medicine and infectious diseases consultants work together in the clinic. We aim to provide holistic care for patients and their families, with support in the clinic from the specialist TB Health Visitor team, and adult and paediatric services running together so parents, children, and other family members needing contact screening can usually be seen in the same visit. TB drugs are supplied directly within the clinic by a specialist pharmacist. There is a weekly MDT before the clinic to discuss new and complex cases, involving respiratory and infectious disease physicians, paediatrician, microbiologist, pharmacist, and TB nurse specialists. We provide a regional service for Multidrug resistant TB and act as a tertiary referral centre for complex mycobacterial disease. The West Yorkshire TB Clinical Network meets twice yearly for cohort review, discussion of complex cases and issues, and TB related CPD. There is a quarterly Leeds TB group meeting to discuss and plan local TB strategy including representatives from LTHT, Leeds Community Health Care, UKHSA, and the City Council. The Leeds COPD Integrated Service The Leeds Integrated COPD Service is a Consultant led Service in collaboration between Respiratory Medicine at Leeds Teaching Hospitals and the Community Respiratory Team at Leeds Community Health Trust. Our aim is to provide accessible, excellent, COPD specialist care across Leeds including quality assured diagnosis & proactive disease management. The service delivers a wide range of activities including delivering COPD care bundle, maintaining best practice standards in acute management of COPD through participation in the National COPD Audit, COPD, Palliative and emphysema MDTs, Home Oxygen Service, Virtual Respiratory Ward in the Community, Alpha 1 - antitrypsin deficiency, Home NIV, Pulmonary Rehabilitation & Specialist COPD outpatient Clinics.

JOB PURPOSE The post holder will be responsible for the safe delivery of direct and indirect care and associated source data collection for clinical research studies undertaken in the department. The post holder will be responsible for the assessment, planning, coordination, implementation and evaluation of individualised care plans for clinical trials participants. Additionally, they will provide patient centred care and support clinical trial participants focusing on high quality patient experience. The post holder will be involved in raising awareness and embedding research within LTHT and offering people opportunities to take part in research as part of their standard care pathway. They will act as a member of the multidisciplinary team, advising and supporting the clinical teams with research related matters and will also maintain their clinical skills. Flexible Working Pattern may be required to support the development of clinical trial services at weekends or evenings that best suit the needs of the research speciality teams and trial participants. Working at different external sites for example Care Homes, Schools or other community settings may be required. During exceptional times of need, they will contribute to clinical service, as agreed with the Head of Nursing/AHP and in line with the Trusts escalation policy. Research Ensure that research studies are undertaken in accordance with study protocols, the terms approved by the Health Research Authority (HRA), National Research Ethics Committee, the Trust Research & Innovation (R&I) Office and all other required regulations. Adhere to national and local Research Governance Framework and legislative requirements to ensure delivery of the highest level of care to research participants. Follow local policies, standard operating procedures (SOPs), and study specific protocols in relation to the research speciality. Provide advice and information to participants in order to facilitate initial and on-going informed consent. This may include obtaining informed consent as delegated by the Principal Investigator and as indicated in the study protocol, and with ethical committee and R&I Department approval, ensuring that the patients rights are upheld throughout the study In line with the Trusts appropriate SOP, additionally training and competency assessment will also be required prior to taking on the delegated responsibility of taking informed consent. Identify, screen and recruit participants into research studies according to inclusion and exclusion criteria, and where necessary, facilitate a participants withdrawal from a study. Maintain effective written and verbal communications with research participants, relatives and other members of the multidisciplinary/ research team to ensure research participant needs are met and appropriate information is shared and documented, including on-going process of informed consent. Have an overall awareness of potential risks within the clinical area assessing these at all times (including patient behaviours and working environment) to ensure the health and safety of research participants, visitors and staff and compliance with related legislation and guidelines. Act as the research participants advocate and uphold the quality of each study at all times; this will include assessment, planning and evaluation of individualised care plans as per study protocol. Report adverse events and serious adverse events in accordance with the Trust SOPs and study protocols in a timely and effective manner with reference to the Senior Research Nurse/Midwife/AHP and Principal Investigator. Resolve complaints timeously at a local level and escalate as appropriate. Be responsible for accurate and secure data collection, storage and entry in to case report forms (both paper and electronic) that meet Data Protection Act requirements, including secure backup of study data, as per information governance and study specific guidance. Participate in facilitating clinical trial monitoring in order to meet the safety, integrity and governance requirements of each study. Where relevant, ensure that the processing and storage of human biological samples meets the necessary requirements of the Human Tissue Act, local SOPs and research protocol so that safe handling and quality is assured. Order and maintain trial specific supplies according to study protocols, procedures and regulations and level of recruitment ensuring that resources are used appropriately and efficiently at all times. Participate in the audit and evaluation of clinical practice in the interests of promoting and improving quality care through action on findings. Have direct involvement in the provision of education and development of pre-registration, appropriate post-registration students and other members of the multidisciplinary team to ensure that appropriate learning opportunities are provided and students feel supported. Have direct involvement in delivering relevant clinical care to Trial participants in accordance with their plan of care.
