SPECIFIC DUTIES & RESPONSIBILITIES: PATIENT/CLIENT CARE To carry a personal clinical caseload to a high level of professional competence and demonstrate clinical expertise in the area of autism and children in Mainstream Schools. To assess, differentially diagnose, plan and implement therapy for children with speech, language and/or communication difficulties on own caseload and supervise others to do so. To work effectively with clients, carers, and colleagues around individual case management; providing advice, guidance and support as necessary and involving them in the planning and prioritisation of care wherever possible. To provide second opinions in your field of clinical expertise to Whittington Health Integrated care team clinical staff on children with complex communicative and emotional disorders. To reflect on clinical practice; adequately identifying strengths & needs of clients, reviewing their response to therapy and providing evidence of sound clinical judgement. To monitor and review the progress of children on caseload and facilitate the development or modification of packages of care / targets as appropriate using evaluation/outcome measurement tools. To adapt practice to meet individual client/carers circumstances, taking into account cultural and linguistic backgrounds. To produce accurate, & detailed reports, therapy programmes, advice and information as necessary in a timely manner. To respond to the requirements of the children and families act 2014 completing written advice which will contribute to Education, Health and Care assessments for children/young people who may have Special Educational Needs. To support the effective transition of students between schools as appropriate. To refer children/young people to other Health, Education or Social Services professionals as necessary. To undertake home visits, as necessary, in compliance with Home Visiting Safety Policy. COMMUNICATION AND RELATIONSHIP SKILLS: To be able to gather information that may be sensitive or complex and analyse it as part of a full assessment of a clients strengths and difficulties. To work closely with clients and carers using empathy, tact, reassurance and active listening to complete assessments, discuss results, and agree treatment options. To routinely overcome barriers to communication during assessment and intervention. This includes the use of sign, symbols or other alternative and augmentation strategies with communication disordered clients/carers, as well as the use of interpreters for clients and carers who do not have English as a first language. To discuss diagnosis and potential outcomes with parents/carers and work collaboratively in decision making regarding referral on and intervention. To motivate difficult to engage clients/carers in the therapeutic process. To negotiate and manage conflict effectively in a range of settings. To deal with initial complaints sensitively, avoiding escalation where possible To form effective therapeutic relationships with clients and/or carers who may be under stress and/or having challenging behaviour. To provide training to a variety of audiences on aspects of speech and language therapy. KNOWLEDGE, TRAINING & EXPERIENCE: To demonstrate highly specialist knowledge underpinned by current evidence based practice and knowledge of national and international research. To identify own training needs as part of the Professional Development Review in discussion with the Lead Therapist for The School and to attend training as agreed. To carry out PDRs for more junior staff To keep up to date with current clinical developments through reading, attendance at courses, meetings and Clinical Excellence Networks(CENs) To maintain up to date professional registration with HPC and RCSLT at all times. To participate in Peer Support Groups. To develop and deliver specialist training informal and formal to others. ANALYTICAL & JUDGEMENTAL SKILLS: To be able to gather and analyse all information, including observation and formal assessment results, in order to identify the most appropriate intervention. To adapt intervention as necessary according to individual need, progress and ability. To provide second opinions to colleagues in complex cases. To demonstrate the ability to reflect on practice and seek appropriate CPD in a variety of settings To develop, record and adapt individual care plans, based on assessment results and response to treatment. To share innovative practice with the wider SLT team To evaluate all training provided and amend according to feedback. PLANNING AND ORGANISATIONAL SKILLS: To manage and prioritise a defined caseload To help others manage and prioritise their workload To plan and organise groups and training, as required. PHYSICAL SKILLS: To demonstrate highly developed auditory skills to enable assessment of speech and/or voice disorders, including a high level of accuracy with phonetic transcriptions. To demonstrate highly developed visual perceptual skills in observations of client interactions and communication To use a keyboard to input data, access emails and produce individualised therapy advice as required. POLICY/SERVICE DEVELOPMENT AND IMPLEMENTATION: To contribute to inter agency team building and policy development. to take a delegated lead on the development of team objectives or projects as required. To develop care protocols for specific clinical areas. To advise line manager of issues around service delivery including staffing issues , service pressures etc To understand and adhere rigorously to the Child Protection procedures as laid down by Whittington Health and multi-agency guidelines. To use specialist knowledge to contribute to service/policy development. To report all complaints/commendations and incidents to the Head of Speech and Language Therapy Services immediately. To prepare responses as required for complaints and attend legal hearings e.g. tribunals. To understand and act upon the complaints procedure as specified by Whittington Health. To be aware of and adhere to all Trust and Service policies To develop and contribute to innovations in the areas of risk management, Quality Standard Setting and Clinical Effectiveness. FINANCE & PHYSICAL RESOURCES To keep and send in accurate statistics within the required time limits. RESEARCH AND DEVELOPMENT: To be innovative in the development of audit, research projects and other clinical effectiveness programmes within your team. To lead in clinical audits on specific projects within your specialist clinical area To design and carry out research projects as required To lead on audits for whole service as directed and complete them independently seeking peer advice as necessary. FREEDOM TO ACT: To manage complex cases independently To be accountable for own professional actions and recognise own professional boundaries, seeking advice as necessary To monitor and evaluate own specialist service delivery, provide progress reports and complete an annual report. To work within defined departmental, professional and organisational protocols, policies and codes of conduct. To plan, implement and evaluate own work, seeking advice as necessary To plan, implement and evaluate others work, alerting the lead therapist for the school to any capacity and capability issues. To participate in monthly supervision with line manager as well as annual performance appraisal in accordance with Whittington health procedures. To supervise others within your field of expertise To support peers in their clinical development by supporting them to develop their clinical expertise. To provide second opinions to colleagues as necessary.
