Clinical: 1. To have responsibility and accountability for the management of own clinical caseload, including referrals, management and discharge decisions, within own level of competence and with appropriate clinical support. 2. To assess and analyse type, severity and impact of all conditions of speech, language and communication impairments in a wide range of adult patients, through a variety of formal and informal assessments/information gathering methods. 3. Main Duties and Responsibilities of the post: The post holder will be responsible for: Supporting the Assistant Chief Nursing Officer Cancer Services and Personalised Care and Support Lead for Cancer administratively Organise, administrate the Cancer Board. The day-to-day administrative support for all the programmes organisation of the various Personalised Care and Support activities, making calls and dealing with enquiries promptly, booking of venues, arranging catering and advertisement of the programmes Supporting the setting up and administration of new and non-routine programmes of activity Attend meetings as appropriate and provide administrative support appropriately preparing agendas, taking minutes and distributing notes of meetings including typing up of group discussions and interviews Support evaluation and data collection for the Personalised Care and Support work streams Communicating verbally and via email with appropriate personnel Inputting data onto the Trust IT systems Managing and updating appropriate Personalised care and support for cancer databases Arranging mail merges and to send out invites, literature, maps and information packs Providing administration support for audit activity when required Ensuring that information is disseminated promptly General clerical and office management duties within the office e.g. own correspondence, supervised mail shots etc. Providing administrative support at the events, courses etc. Working together with other administrators/PAs within the Trust to provide an effective network of communication Liaise and work with the other cancer transformation programme coordinators in the other Trusts and in the Somerset, Wiltshire, Avon and Gloucestershire Cancer Alliance.

To communicate appropriately with all parents/ carers, visitors, and staff. This may include adults with special needs, culture and language differences and/or challenging behaviour. Provide an efficient reception service Provide a comprehensive telephone enquiry service for the clinical area. This will include receiving telephone calls from families who require regular updates on their babies admitted to the unit, applying the relevant security checks To use initiative, judgement and decision-making skills in situations that arise on the unit daily and report any issues that may affect patient care and/or the service. Admit and discharge patients on the computerised hospital system, ensuring accurate patient details are recorded. Retrieve and return medical notes and x-rays as required. Organise medical notes files following the correct protocol. Assist in the provision of the required paperwork and medical notes for planned and/or emergency admissions. Book outpatient and other appointments, or investigations for patients as required. Sort and distribute incoming and outgoing mail for the unit. Refill the supplies of stationery for the reception area as required. Order, receive, store and distribute clerical supplies according to requirements. Establish formal and informal lines of communication with parents, carers, visitors, and staff. Liaise with other members of the clinical and reception team as required. Take full responsibility for developing own practice and training needs. Attend team meetings and appraisal/performance reviews. Plan own workload and prioritise need. Take on other such reasonable duties as appropriate, as directed by the Nurse in Charge of the unit or Line Manager. Adhere to Safeguarding procedures Contribute to the provision of a learning environment for staff undergoing training. Under the guidance and supervision of the Cancer Clinical Nurse Specialists and Haematology Support/Specialty Managers, assist in the coordination of patient care throughout the pathway: To provide general information and support about cancer and cancer services, to enable people to navigate the health and social care system and make choices that are best for their cancer and their life. A key aspect of the role involves daily and direct (e.g. face to face and telephone) communication with patients, relatives, carers, and other health and social care professionals. The information and nature of the communication required is sensitive due to the nature of cancer. Communication in this context requires a high degree of empathy, understanding, diplomacy, honesty and integrity. Triage patients, using a risk assessment framework and initiate appropriate response according to protocols and individual pathways, using good communication skills, basic clinical awareness and appropriate tools and procedures, liaising as appropriate when non routine and refer complex decisions to the team for assessment and review Provide basic telephone and face to face advice and refer on or sign-post to other sources of support Provide support to patients in the clinic setting when required Coordinate the necessary assessments, appointments or investigations to fast track people back into the system if required Demonstrate the ability to recognise and respond appropriately when faced with a sudden deterioration or an emergency situation, alerting the team or enabling rapid response as appropriate Support information provision and signpost as appropriate Guide people through the use of self-assessment resources Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit To proactively identify patient and carer needs using approved tools and procedures to ensure that people get the right support to meet their needs. Ensure all outcomes of the MDT meetings, tests, investigations, and referrals are recorded and acted upon. To be proficient in the use of multiple hospital IT systems, including CRIS, CareFlow, ICE and Somerset Cancer Register. Support the Haematology administration team with typing and booking to ensure cancer patients are not delayed due to administration delays. Carry out some clerical duties to support the Clinical Nurse Specialists.

The post holder will: Support teams to ensure that the portfolio of tasks/projects is planned, managed and delivered effectively. Support and inform the targeting of resources, monitoring spends, implementation and evaluation of the tasks/projects by providing high quality support including complex in formation and analysis, communications and stakeholder management Ensure accurate and open communication and co-ordination with a range of organisations and individuals, researching and drafting correspondence and papers and ensuring the management of specific tasks, lead reporting and analysis across a range of specialties, functions and projects.

In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. Making professional, autonomous decision in relation to pesenting problems, whether self-referred or referred from other helth care workers within the organisation.
