This nurse provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. The focus is on individual growth and development in outpatient PACT nursing practice.

Under the supervision of the Nurse Manager/Charge Nurse/Team Leader, the Licensed Practical Nurse (LPN) provides nursing care to the Veteran patient with consideration to psychosocial, cultural, and age specific needs. As a Staff Nurse you will provide prescribed medical treatment and personal care services to ill and injured veterans. Duties include documenting observations, assessments, and changes in patient's condition; collaborating with health team members to facilitate positive patient care outcomes; providing a full range of nursing care to patients with a variety of physical and/or behavior problems; and accountable for meeting and maintaining the qualification standards and functional statements of assigned grade. Major duties and responsibilities will include:

• This position has the responsibility to attend to the immediate needs/questions of our patients, their families and visitors. However, the incumbent is not a clinical subject matter expert, case worker, counselor, or clinician.

The MSA is responsible for scheduling appointments, including interpreting, and verifying provider orders in accordance with VHA national scheduling guidelines.
