POSITION SUMMARY
This position provides comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care. CORE FUNCTIONS

6. Supports the patient and family during difficult decision-making periods. Assist in coordination of end of life care for patient and family and provides emotional support.

7. Supports process improvement activities for populations of patients to achieve the optimal clinical, financial, operational, and satisfaction outcomes. Participates in staff development to maintain current standards of practice and ensure the highest quality of care.

8. Establishes and promotes a collaborative relationship with physicians, payers, and other members of the health care team. Collects and communicates pertinent, timely information to payers and others to fulfill utilization and regulatory requirements.

9. This position works independently, with freedom to determine how to best accomplish functions within established procedures. This position confers with supervisor on any unusual situations. This position is facility-based with no budgetary responsibility. Internal customers include all levels of staff, including physicians. External customers include patients, families, employees of other healthcare institutions, physician offices, referring physicians, community providers and agencies, payers, provider networks, and regulatory agencies. This position requires making decisions related to patient care coordination throughout various facilities and with various specialty practices. Decision making required for treatment and standardization of care of patients. This position requires complex problem solving related to plan of care for evaluation, treatment, and discharge planning for patients.

MINIMUM QUALIFICATIONS
Must possess knowledge as normally obtained through the completion of a Bachelor's degree in nursing or related field.

Depending on area of focus, requires a current RN licensure, PT licensure, or OT licensure in the state of practice. BLS certification required.

Requires a proficiency level typically achieved with 5 years clinical experience. Requires excellent organizational skills and clinical knowledge regarding specialty care services, as well as care coordination of services, legal and financial aspects of diagnostic services and health services in specialty area. Requires effective communication and writing skills, good time management skills and knowledge of word processing and database software applications. Requires the ability to teach both clinical and non-clinical personnel regarding care and diagnostics services. Also requires a good understanding of process improvement. POSITION SUMMARY
This position performs a variety of support functions, under the supervision of clinical pharmacists, to contribute to the overall improvement of healthcare quality as well as efficient use of resources. Primary responsibilities include processing prescription renewal requests received electronically in the electronic health record, via fax, or phone call; data management and pre-screening to support pharmacist disease state management services, including patient outreach to enroll patients in the service; and patient outreach to assist with closing quality measure care gaps. This position assists in daily operations of the Medical Imaging Department under direct supervision with technologists in the department.

As an MIA you will help organize work flow, assist the Technologist with transferring patients to exam table, cleaning workspaces, stocking rooms, transporting patients, if necessary, all while having face to face contact with patients. POSITION SUMMARY
This position assists technologists and/or nurses in department by performing non-technical, patient and procedure-related duties in accordance with department/facility policies and procedures. This position is also responsible for a variety of clerical and administrative support functions and customer service for an assigned patient care area which promote an organized, efficient delivery of care to patients.

CORE FUNCTIONS
1. Performs a variety of tasks and functions associated with the department’s daily operations, including but not limited to, greeting patients, medical staff and the public, answering incoming telephone calls, scheduling appointments, entering orders and providing general information to customers. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution. Refers complex inquiries to the appropriate department or staff. Provides ongoing clerical and administrative support.

2. Enters patient information into the computer, scans documents into PACS. Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient’s record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. May be responsible for QC/validations. Assists in facilitating requests for patient medical records.

3. Provides support in the acquisition and distribution of digital patient records. Digitizes, formats, and uploads film-based or digital images into PACS. Maintains accurate filling system as required based on established department protocols and procedures.

4. Fulfills requests for images and reports ensuring availability in system, hard copy (film), and/or CD as required and follows proper protocols for releasing confidential patient information.

5. Receives patient in applicable systems, confirms patient demographics and insurance information and performs insurance verification when required. May be required to calculate and collect copays, deductibles, and self-pay. Totals charges on patient’s super-bill at check-out. Enters pending discharges into the system. Coordinates patient appointments for procedures and treatments that may be required prior to timely discharge. Processes patient discharges. Coordinates transportation to care centers and residential homes as requested by patients or physicians.

6. Prints physician schedules, super-bills, labels and other applicable forms in preparation for next business day. Pulls and files patient charts and prepares new patient charts as needed. Prints and files transcription notes. Reconciles charge tickets to each day's schedule, identifies incomplete tickets, missing charge codes or missing diagnosis and notifies clinical staff. Forwards completed tickets and/or billing package to the business office or billing company.

7. Assists technologists and nurses in pre and post procedure room preparation by obtaining necessary supplies and assisting patients as necessary. Ensures procedure rooms are stocked appropriately. Understand sterile procedure and assists with setting up sterile trays, where applicable. Adheres to radiation safety policies. Escorts patients back to the patient exam rooms, takes vitals and patient history; may assist with patient treatment to include peripheral IV insertion, IV discontinuation and Point of Care Testing (POCT) if required by the assigned facility (completion of Banner’s peripheral IV insertion course and validation of skill must be completed prior to any peripheral IV insertion and discontinuation performed. Completion of Banner’s POCT education and validation of skill must be completed prior to any Point of Care Testing being performed).

8. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery.

9. This position assists in the operations of the medical imaging department under direct supervision with technologists and nurses in department. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Must navigate a variety of separate computer programs at any given time and handle multiple demands on time. Internal customers: All levels of medical imaging and nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: regulatory and health agencies, non-clinic providers and facilities, vendors, patients and family members.

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge.

Certification for BLS is required.

Must be able to exercise good judgment, effectively communicate, and use discretion with patients, staff and the general public. Working knowledge of medical terminology and computer literacy is required. The position requires knowledge of commonly used office software and keyboarding skills. Good organizational skills are essential in prioritizing and coordinating various responsibilities simultaneously. POSITION SUMMARY
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. POSITION SUMMARY

5. Develops and continually updates work processes to meet changing and growing facility needs for supply chain services. Makes recommendations, devises, implements, and enforces department policies and procedures in conjunction with other members of the leadership team. Works as a member of the system/region materials leadership team to develop and standardize efficient and effective practices. May be responsible for ensuring that staff is fully trained and certified in forklift equipment operation as well as fully educated and held accountable for all safety procedures.

6. Maintains, updates, researches, and tracks shipping/receiving records for departments, Systems Materials Management, and Accounts Payable. Maintains department records relating to personnel matters, in-service education, accounts payable, supplies received, equipment maintenance, and other related areas.

7. Departmental/facility responsibility. Incumbent assists all departments with ordering, shipping and, receiving functions. Works under limited supervision using independent judgment to achieve goals. Internal customers are other staff, nurses, physicians, patients, visitors, supervisors, managers and executives. External customers are patients, visitors, vendors, other company facilities, and non-company medical facilities.

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge.

Must posses a valid driver’s license and be eligible for coverage under the company auto insurance policy.

Requires the skills, knowledge and experience acquired over three or more years of computerized inventory management and shipping and receiving functions, plus extensive knowledge of healthcare supplies and handling processes

Requires sound math and business skills, intermediate abilities in word processing, spreadsheet and/or database software use, and the ability to research and compile statistical reports. May be required to be forklift certified within 30 days in certain facilities. The salary offered to a selected candidate will be based on multiple factors including location, hire grade, job-related knowledge, skills, experience, and with consideration of internal equity of our current team members. In addition to a comprehensive benefits package, candidates may be eligible for other forms of compensation such as participation in a bonus and a stock award program, as applicable.

For all sales roles, the posted salary range is the Target Total Cash (TTC) range for the role, which is the sum of base salary and target bonus amount at 100% goal achievement.

Position Summary...

What you'll do...

Develops and supports Membership by providing information on Membership benefits, promoting the value of Company products and services, processing Memberships, upgrades, and credit accounts, participating in sales and marketing events, and making efforts to meet Membership goals.

Maintains safety of Facility by following all safety standards, procedures, and guidelines including conducting safety sweeps, following proper forklift spotting procedures, following proper procedures for handling and disposing of hazardous materials, following Company steel standard guidelines, and correcting/reporting unsafe situations to Management.

Fulfills Member Fax 'n Pull and Click 'n Pull orders by reviewing orders, pulling items from shelves, scanning items, wrapping and palletizing items, ensuring the accuracy of orders and invoices, securing items until Members arrive, and following up on out-of-stock merchandise to meet Member needs.

Completes work assignments and priorities by using policies, data, and resources; collaborating with managers, co-workers, customers, and other business partners; identifying priorities, deadlines, and expectations; carrying out tasks; communicating progress and information; determining and recommending ways to address improvement opportunities; and adapting to and learning from change, difficulties, and feedback.

Provides Member service by acknowledging the Member, identifying their needs, assisting with purchasing decisions, locating merchandise, resolving issues and concerns, and promoting the Company's products and services. Complies with company policies, procedures, and standards of ethics and integrity by implementing related action plans; using the Open Door Policy; and applying these in executing business processes and practices.
