The Crisis Assessment Practitioner role will involve carrying out rapid clinical risk and mental health assessments, formulation, short-term contingency planning and onward referral for people presenting in mental health crisis in the borough of Lambeth. Assessments typically take place in people’s homes, however can be at team base or GP surgeries.

The service operates Monday to Friday 9-5pm currently however there is a proposal to extend working hours to include late evening and weekend cover.

Working within the community, the Crisis Assessment Practitioner will provide face-to-face assessments and work closely with other mental health services and agencies across the borough of Lambeth, including stakeholders in the Lambeth Alliance. They will act as a trusted assessor for SLaM and decide on the most appropriate crisis response and treatment pathway for people contacting the Crisis Outreach Service, in collaboration with the Band 7 Advanced Practitioner in the team.

Using a range of NICE recommended approaches including brief solution-focused interventions, Crisis Assessment Practitioners in the Crisis Outreach Service will empower patients, carers and their families in decision making and to access a range of health and social care resources for support.

The Band 4 Stroke Reviewer, will work under the direction of the Band 5 Senior Stroke Reviewer, Deputy Clinical Lead for Community Stroke Team and other health professional colleagues, including Occupational Therapists, Speech & Language Therapists, Physiotherapists, Neuro Psychology, Nursing and Stroke Care Advisor as part of a busy community Stroke rehabilitation multi-disciplinary team.

To work across multiple community sites within the boroughs of Lambeth and Southwark, including GSTT designated community sites for example Pulross Centre, Tessa Jowell Health Centre, and Bermondsey Health Centre and service users place of residence.

The post holder will carry out comprehensive stroke review assessments to our service users approximately 6 months post stroke using a validated and standardised tool.

The role involves analysing the needs of the service user from information on electronic health records, collateral history from the service user, their relatives and/or carers and from the Stroke review.

To then identify new or unmet needs relating to the service user’s Stroke and determine appropriate actions to address these needs.

The post holder will work autonomously and be responsible for their own busy caseload in order to meet the individual needs of adults living with physical and/or cognitive and communication disabilities following Stroke.

The Stroke Reviewer will also support service users’ carers in finding suitable ways of conducting their role and identifying areas in which they might need support.

Following assessment, a summary of the review will be sent to the service user’s GP, inclusive of concerns and/or recommendations.

Clinical Duties

To be responsible for contacting service users eligible for a Stroke six month review and to offer an appointment either by phone/virtually, in their home or at clinic.

Without direct supervision to independently carry out an assessment for Stroke survivors using a validated and standardised tool, developing a particular focus on new needs the service user may have developed since their discharge.

To provide Stroke prevention education, advice and guidance to improve self-management and encourage meaningful engagement within their local community.

To liaise with service user’s carers where applicable to explore any needs that they have which should be addressed. Clinically reasoning to determine appropriate onward referrals / care pathways to assist the service user’s carer/s.

To sensitively deal with distressing or emotional circumstances, that may include resolution of conflict regarding patient care and to provide an empathetic approach utilising counselling skills.

Communicate sensitive and complex information regarding rehabilitation to service users, carers, family and other health and social care professionals and organisations.

To screen a service users or carer’s psychological state using validated mood screens and to offer advice for onward support where needed. To also complete the GSTT suicidal risk assessment when indicated and notify service user’s GP and/or the Community Stroke Team Clinical Psychologist for urgent mood review when indicated.

To complete medication reconciliation to ensure service users are taking the recommended prescription as stated on their hospital discharge report. This includes identifying secondary Stroke prevention medications and notifying the GP or Community Pharmacist of any concerns identified.

To review walking aids and equipment that has been prescribed previously by clinicians to ensure that they continue to meet the patient’s moving and handling needs in facilitation function. If not, to determine appropriate onward referral that is required to review or address unmet equipment needs.

To write a report to the service users GP highlighting any concerns identified and rationale for actions that need to be completed by the GP i.e. medication or blood pressure review etc.

Using analytical and clinical reasoning skills make referrals to other services within the community as per the service user’s needs. This may involve extensive liaison with the multi-disciplinary team, GSTT and / or Local Authority Safeguarding Teams, as well as external organisations i.e. social services and the charity sector for advice on onward referrals.

When relevant complete follow-up actions by contacting the service user with leaflets and information and to provide advice when needed for secondary stroke prevention.

To maintain adequate electronic health records including clinical notes as per service standard operating procedures.

To seek timely advice and support as appropriate from the Senior 6 month Stroke Reviewer.

To participate in the triage of referrals, using appropriate rationale to prioritise accordingly.

To effectively manage own varied caseload, using prioritisation and time management skills in a variety of community settings and often without direct supervision. To ensure clinics are appropriately booked in order to maximise delivery of appropriate and effective care, liaising closely with senior staff to achieve this where required

To demonstrate therapeutic handling of patients, for example positioning during assessment of relevant clinical observations complying with the Trusts Manual Handling Policy and local therapeutic handling guidelines.

To carry out risk assessments (including manual handling) and ensure actions are taken for safe intervention as appropriate, complying at all times with requirements of health and safety regulations.

To complete blood pressure monitoring and pulse oximeter to measure the percentage of the service user’s oxygen saturation levels. To also recognise the appropriate cuff size needed to measure blood pressure and report any abnormal readings to the service user’s GP.

To communicate either verbally or written to a high standard and share information effectively with all members of the multidisciplinary health and social care team, referrers and external agencies regarding information related to their work with the service user whilst observing data protection and confidentiality guidelines.

To represent the department in internal and external meetings as appropriate and to take part in training and mentoring as and when required.

To respond to crisis and safeguarding incidents effectively, seeking advice from the Senior Stroke Reviewer or Community Stroke Team Deputy Clinical Lead as required, and escalating concerns where appropriate.

Be proactive in developing strong partnerships with local agencies and community groups to create a network of resources where appropriate. To also participate in ensuring an up-to date information resource on services is available to the wider team that can support service users to access mainstream activities and social groups.

To support the Lambeth Stroke Care Advisor by delivering peer support groups for our service users by recruiting participants as well as sourcing speakers.

To share in the responsibility for the security, care and maintenance of equipment ensuring standards of infection control and safety are maintained. Undertaking routine and general light cleaning / tidying tasks within department clinical areas.

To have specific responsibility for keeping communication aids in working order and ready for use. This includes a work mobile, tablet/laptop and a SkyGuard device. To wear a name badge at all times.
