Duties included, but not limited to:
- The Licensed Practical Nurse works under the direction of a Registered Nurse (RN) and reports to the primary care nurse manager. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN is a member of the patient healthcare Teamlet.
- The LPN requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures. The LPN demonstrates active collaboration and communication with Teamlet (Patient, Primary Care Provider, RN Care Manager, Clinical Associate and Clerical Associate) and expanded Team (family/caregiver, internal and community based services and disciplines involved in providing care to the patient).
- Assists the Registered Nurse in the coordination of patient care activities with other departments by preparing patients physically for scheduled tests/procedures. Makes observations and thoroughly documents care and the patient's response.
- Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Monitors patient during the procedure.
- Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular) and performs treatments according to established policies and procedures.
- Observes for changes in patient condition, caused by medications, treatments, and procedures.
- Records and reports these observations and reports any deviation from normal to the MD/PA/NP/RN.
- Accurately documents prescribed medications/treatments according to established policies and procedures.
- Documents patient's responses to medications/treatments received.
- Preforms patient blood collection, when necessary, utilizing proper selection and use of equipment/supplies.
- Follows all policies and procedures regarding appropriate specimen collection, laboratory results, critical values/parameters and follow up protocols in collaboration with providers.
- Participates in performance improvement activities.
- Completes unresolved clinical reminders on patients who present to their area for check-in.
- Consistently communicates with patients/families/co-workers and the general public in a courteous, tactful and helpful manner. Both oral and written communications are provided promptly and are responsive to the customer's needs.
- Appropriately seeks assistance and guidance by informing RN or medical provider of changes in the patient's condition requiring higher-level intervention.
- The LPN performs, reports and documents assigned nursing care in the Primary Care Clinics, but not limited to:
- Oxygen therapy
- Medication administration to include IV medication if certified.
- Clinical reminders with appropriate consults and alerts as indicated
- Check expiration dates of meds and supplies
- Delegated procedures and treatments
- Documentation
- Assists with minor procedures, including obtaining and arranging necessary
- Adheres to standards of practice and safe techniques in preparation and administration of medication.
- Demonstrates understanding of age-and population-specific characteristics that affect the health care needs of the Veteran.
- Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes as directed when appropriate.
