We’re recruiting a Senior Accommodation Support Officer to provide support to our established team as they assist our participants with their housing needs through effective interventions.



Our team of Accommodation Support Officers work with a caseload of adult males within the criminal justice system, in either a custodial or community setting. As a senior, you’ll be on hand to provide advice and guidance to the team where needed, and we’ll ask you to take care of a small amount of our more complex cases.



We’ll need your excellent interpersonal skills to build and maintain effective relationships with referral agencies and stakeholders. You’ll also be responsible for developing high-quality practice among the team through observing and auditing practice, and then providing coaching and mentoring workshops to enable them to develop.



Our successful candidate will ideally already have knowledge of housing and homelessness legislation or experience working within the criminal justice sector.

The Role:

In the role of a Service Delivery Manager, you will be in control of the SEN Transport and Social Care Transport Operation, including a team of 7 officers and a Principal officer. The role reports directly to the Head of Passenger Transport and Fleet
As a Service Delivery Manager, you will work on schedules and processes to ensure service users safe and efficient carriage as well as liaising with contractors, service users, parents and carers, schools, daycentres and parent forums regarding service delivery
Other management responsibilities can include ensuring all controls are in place regarding safe transport such as risk assessments and relevant equipment such as harnesses and child seats in use if appropriate along with undertaking investigatory or disciplinary hearings and grievances
As a member of the SEN and Social Care Transport team, you must comply with current legislation and policies; ensuring staff and contractors meet required safeguarding and DBS requirements as well as monitoring and updating KPIs and financial information
The Service Delivery Manager will work closely with stakeholders, including other departments, Parent & Carer forums, Transformation team and respond to complaints, appeals, freedom of information or SAR requests related to the department and will ensure invoices and spend is accurately controlled and reported

We are recruiting for a Healthcare Assistant to join our community-based team in Wirral, North-West. Hours:Bank (minimum of 37.5 hours per month) for our night shift team. Base:within 25 miles of your home address. Travelling to the Liverpool Hospice may be required from time-to-time. In this role, you will work alongside a multidisciplinary team to provide assessment, care planning, review and evaluation of holistic care management plans individual to each patient. As this is a community-based role, you will be responsible for planning your own work, responding to changes in patient conditions, prioritising care needs (including hygiene, nutrition and hydration, comfort, monitoring vitals, and more) as detailed in the personalised care plan and ensuring you escalate appropriately to senior staff within the multidisciplinary team in a timely manner. What were looking for: National Vocational Qualification (NVQ) Level 2 in Health and Social Care OR an equivalent alternative Level 2 Qualification. Minimum of 6 months care delivery experience in a Health or Social Care Environment. Clinical Nurse Specialist - Adult

Due to our expansion in our Clinical Services we are seeking to appoint an enthusiastic and experienced individual to AdultCommunity Nurse. This is a fantastic opportunity to utilise your skills and provide exceptional healthcare to our clients and their families. This is a new role in the area and a unique opportunity to create and establish our Clinical services in the area.

You'll be working with our Carers and complex care patients in their own homes within the community, ensuring that Carers are suitably trained and competent to deliver the care needed. You'll assess the risks and needs of the patient to devise an individual holistic detailed electronic care plans and that they are regularly evaluated to ensure that patients receive supervised evidence based nursing care in accordance with local contracts.

You will be responsible for managing your own caseload and therefore your own diary whilst typically working Monday to Friday 09.00 - 17.00.

What we need from you

- Current registration to practice

- Clinical experience including Tracheostomies, ventilators, stoma bags etc

- Ideally, previous knowledge and/or experience in working in a complex care setting or community setting

- Excellent communication skills and ability to be able to train clinical and non-clinical staff

- Submit clinical reports and updates to CCGs regarding their packages as per contractual agreements

- Maintain clinical updates and evidence of PREP (Post Registration Educational Portfolio)

- Ideally have leadership skills/experience

- Driving licence and use of a vehicle for work
