This Nurse Practitioner position is located in Home Based Primary Care at the VA Manchester location. This position is full-time at 40 hours per week.
Included but not limited to:
- Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
- Collaborates with patients, families, and the health care team to develop and implement treatment plans and evaluate health outcomes.
- Demonstrates critical thinking in assessing and managing complex clinical situations.
- Working collaboratively with other PACT and HBPC members.
- Participates effectively in team meetings and treatment planning conferences and collaborates with IDT members in a manner that enhances coordination of comprehensive Veteran-centered care.
- Assist as requested with clinical procedures.
- Act as the focal point of the designated outpatient clinic by being the contact person for all other consultant-specialty services regarding patient care.
- Initiate effective actions in emergencies.
- Manage prescriptions for assigned patients.
- Work effectively and make appropriate requests for assistance from collaborating physician as appropriate.
- Perform appropriate evaluations, diagnosis, and treatment of patients within HBPC.
- Orders diagnostic tests and discriminates between normal/abnormal findings and makes appropriate treatment decisions based on available information.
- Responsible for timely completion of all clinical charting and documentation required by the medical Center or healthcare system, as appropriate.
- Involved in performance improvement and quality assurance initiatives.
- Should be a forward-thinking provider who embraces Veteran-centered geriatrics, palliative, and hospice care delivery.
- Initiates referrals and consults to specialty services.
- Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.

Work Schedule: Full time, Monday- Friday 8:00am-4:30pm
