The Analytics and Performance team is a growing function within the AASC contract. The department will be responsible for producing accurate, current, and insightful Management information to provide all contractual, external and internal functional, performance and insight reporting. The team is also responsible for optimising service performance, driving adoption, effective business change, and ultimately helping to develop a data-driven organisation. This function is fundamental to the day-to-day operation of the AASC contract and is critical in understanding and improving the business operations. Main purpose:

Main duties of the job: Clinical 1. The direct provision of high-quality evidence based person-centred physical and mental health care to service users within the adult ADHD service. 2. To conduct remote ADHD assessments using evidence-based assessment tools and techniques. 3. To work as an independent non-medical prescriber taking responsibility for the clinical assessment, establishing a diagnosis, and discussing all available treatment options (including non-pharmacological treatment) as per NICE guidance. 4. Generate clear, accurate and timely reports summarising the results of the assessment and providing recommendations for ongoing treatment and support. 5. To prescribe and monitor medications in accordance with Pan Mersey formulary and prescribing guidance, including the regular monitoring of effectiveness, concordance and adverse effects of medication, tailoring treatment where indicated. 6. Stay up to date with professional qualifications including revalidation of certificates and registration. 7. To ensure a holistic approach to assessments and treatment is undertaken, including consideration of carer needs, physical health needs and signposting to other resources as required, continuing to consider these needs when developing each service users ongoing care plan. Support the Medical Director to run their office efficiently, supporting with Day to day diary management for the MD, using Microsoft Outlook and NHS Calendars: add, amend, delete appointments, send invites, book rooms and venues and forward plan. in assisting and empowering residents to maintain independent living.




This is a permanent post based at Harpers Green in Warrington, one of our purpose-built extra care schemes commissioned by the
local authority to deliver 24/7 care and support to people aged 55+.




The facility has a dedicated Torus housing team who work collaboratively with the care provider to ensure all residents are able
to access opportunities to actively engaged in social activities and events on their schemes and in their local communities. The
scheme includes commercially operated bistro and salon which offers an exciting opportunity for a local business to deliver
services to residents and people in the surrounding areas.




Responsibilities:

* Carry out viewings of properties with applicants and potential applicants, promoting the benefits of extra care services.
* Develop personal plans and agree interventions that enable residents to manage their tenancies safely and successfully.
* Assist residents with settling in and maintaining their tenancy, including understanding their rights and responsibilities as
residents.
* Assist residents in accessing a full range of universal and preventative services.
* Make the appropriate referrals and demonstrate positive multi-agency working with the relevant agencies when there is a change
