The Dialysis Registered Nurse (RN) is a professional nurse with specialized knowledge and demonstrates advanced beginner skills when providing and directing care for patients with end-stage renal disease (ESRD), acute kidney injury (AKI), and late-stage chronic kidney disease (CKD). Collaborates with other disciplines/departments to achieve desired patient outcomes. Functions as primary nurse for ESRD patient panel and serves as key resource assisting patients navigate the healthcare system. Completes documentation for all clinical and technical recordkeeping in the electronic health record and treatment flowsheets in accordance with established policies and procedures. Monitors, tracks/trends, and reports ongoing care continuity needs and outcomes to the interdisciplinary team. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Practice includes directing other providers. Partners with the patient, family, and interdisciplinary team to promote effective and safe transitions across the health care continuum. This nurse has fundamental experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice. The job duties of the Dialysis RN include but are not limited to-
- Applies advanced clinical knowledge/judgment to lead staff involvement in planning, decision-making, and evaluating outcomes beyond the immediate practice setting
- Leads and organizes delivery of care to ensure continuity of care and peer accountability for practice in alignment with the ANA Standards of Nursing Practice
- Leads initiatives to improve the Veteran experience i.e., care coordination, timeliness to resolve issues, negative trends, or environment of care concerns, beyond the immediate practice setting
- Models' high reliability principles and I CARE values to improve the consistency of care delivery and patient outcomes beyond the immediate practice setting
- Demonstrates positive, effective communication skills, professional behaviors and leadership that promotes a commitment to excellence, improving nursing practice, and fostering collaboration among customers beyond the immediate practice setting
- Makes sustained contributions to healthcare by actively participating in healthcare facility or service-level committees or councils, establishing active relationships and demonstrating leadership in improving care beyond the immediate practice setting
- Advances nursing practice through contributions to knowledge development or education beyond the immediate practice setting
- Leads educational initiatives that expand knowledge to improve outcomes beyond the immediate practice setting
- Demonstrates expertise in managing resources that contribute to efficient, costeffective care and, sharing knowledge beyond the immediate practice setting
- Demonstrates expertise in evidence-based practice/research resulting in improved outcomes beyond the immediate practice setting
