We are looking for someone who is passionate about teaching Music to join us at Kirton Academy. The successful candidate will have their own custom designed designated Music teaching room with attached practice / peri rooms.

Leading a county wide service to provide vaccinations and ensure a joined-up communicable disease prevention, treatment and vaccination offer in line with the National Vaccination Strategy. A senior lead role responsible and accountable for the clinical
leadership of the whole vaccination process, trained and competent in
all aspects of vaccination, immunisations, communicable disease response and care.
Responsible for clinical oversight of the following:
Coordination and oversight of the full portfolio of Communicable Disease Response Team services working closely with TB services and the School-Aged Immunisation Service.

Provision of adequate leadership and support to all staff and citizens.
Clinical escalations from clinical supervisors, clinical assessors and
vaccination staff.

Here in the Discharge to Assess Pathway 1 Team we have fantastic opportunities for therapists looking for the next step in their career.

We have 4 small friendly teams across Lincolnshire comprising of Occupational Therapists, Physiotherapists, Nurses, Pharmacy Technician, Promoting Independence Therapy Assistants and Promoting Independence Care Assistants. We are a growing team and looking for enthusiastic, friendly, and proactive therapists.

In our Pathway 1 Team, we follow the patient home and see them within 2 hours of their discharge. We listen to the patient of what matters most to them with regard to their personal and social circumstances, their physical, mental and emotional health and their carer’s needs. This will include exercise programmes for their physical and mental wellbeing, recommending and providing the correct equipment, proactive work with patients to encourage independent living, educating patients on self-management of their health and wellbeing such understanding frailty symptoms and the importance of staying well physically and mentally for falls prevention, and so much more.

The Pathway 1 Discharge to Assess Service provides therapy based rehabilitation and reablement to patients immediately following hospital discharge. It will be the role of the therapists to provide profession specific assessments, create rehabilitation care plans and to work closely with the team of Advanced Therapy Assistant Practitioners, Promoting Independence Therapy Assistants and Promoting Independence Care Assistants to ensure patients are seen and reviewed in a timely manner to maximise recovery. To work closely with all members of the multi-disciplinary team and to work at times in conjunction with other agencies to determine a person’s long term care needs.
