This position is for Population Health Campus (National Healthcare Group).

Job Responsibilities

• Work with the group of general practitioners (GPs) in the network to provide structured nurse counselling, health screening services, patient education on chronic disease, mental health management and health promotion through face-to-face or virtual consultations
• Identify patients suitable for nurse counselling to help them understand their chronic conditions and improve their health outcomes
• Assess patients’ social history, current lifestyle and dietary habits to help patients bring about effective change and enhance their well-being
• Work closely with the GPs in providing individualized care plans to patients to manage their chronic conditions effectively
• Teach/advise patients on how to administer insulin injections, perform self-monitoring of glucose and home blood pressure readings, proper usage of inhalers, provision of targeted diet plans and other nurse-related counseling
• Conduct eye and foot screening with patient education to diabetic patients and perform spirometry test to diagnose patients with asthma or COPD
• Assess patients’ mental well-being through questionnaires and escalating to the GPs when required.
• Provide support to the admin team in tracking and monitoring of the clinical and outcome indicators for the clinics
• Work closely with the Primary Care Coordinators (PCC) on scheduling patients for ancillary services
• Support the PCN in quality improvement projects to achieve better clinical and process outcomes
• Perform any other duties assigned by the Reporting Officer

Job Requirements

• At least an Advanced Diploma in Nursing
• Registered Nurse with the Singapore Nursing Board
• Specialist Diploma in Diabetes Education/Gerontology or Advanced Diploma in Chronic Disease Management is preferred
• Minimum 4-5 years of working experience, with experience in chronic disease management preferred
• Experience in conducting health screening and provide comprehensive health education to patients with chronic conditions
• Ability to provide good clinical judgment, possesses critical thinking and problem-solving skills in the delivery of patient care in line with established work instructions and protocols
• Excellent communication and interpersonal skills are essential
• Passionate about making a difference in improving the health of patients
• Possess proactive attitude, team spirit and maturity
• Meticulous and able to keep proper and timely documentation
• Conversant in a second language/dialect would be advantage to speak with patients
• Willing to travel to the northern and central parts of Singapore

PRINCIPAL RESPONSIBILITIES

Calendar & Scheduling

• Provide secretarial support to the Director, Clinical Research Unit (CRU)
• Manage appointment scheduling of the Director’s Calendar
• Process claims and payments as necessary
• Provide clerical support
• Assist with travel arrangements for the Director and other team members on CRU work-related trips

Administrative Support

1.Mails

1. Collect, sort and distribute in-coming mails, faxes and circulars
2. Despatch out-going mails e.g. pamphlets, notices, letters, circulars and publications to polyclinics / doctors through mail room
3. Fax, file, photocopy, franking letters and retrieve department records and essential correspondences

2. Meeting Arrangements

1. Co-ordinate CRU departmental meetings including dates of meetings, booking of meeting rooms, gathering of agenda and timely dissemination of minutes to team of CRU, and other meeting attendees where necessary
2. Arrange other ad-hoc meetings and refreshment when required.
3. Schedule meetings for the department and send reminders to parties concern.
4. Preparation of rooms and other logistics (eg. attendances, printing of handouts when necessary) for meetings and events
5. Writes minutes for meetings upon request

3. Data collation

1. Collate and maintain statistics upon request
2. Submit collated training events’ attendances to NHGP HRD after training events when necessary

4. Research-related Administration

1. Provide administrative or logistic support to Manager and CRU team upon request
2. Provide grant administration support as necessary
3. Assist the Director, CRU to prepare presentation slides upon request
4. Assist Manager, CRU to organise research-related events such as Journal Clubs, Research Didactics and Forums
5. Assist Manager, CRU to organize and coordinate training courses and seminars in research and medical ethics
6. Assist Manager, CRU to provide secretariat support to various research initiatives e.g. NHGP Research Attachment, CRU Research Assistance applications
7. Track research outcome
8. Assist in maintaining research training records
9. Coordinate and collate for research-related course registration for doctors
10. Renewal of research-related E-journals

5. Other administrative matters

1. Tracking of leave (eg. Annual Leave, Medical Leave) & maintain staff movement chart where necessary
2. Raise IT Job Request when necessary
3. Purchase stationery items for departmental use and maintain tidiness of stationery storage
4. Raise Purchase Requisition and e-Payment and other modes of payment when necessary
5. Maintain and back-up a good filing system in the department and shared drive, including maintenance of CRU assets and IT equipment used in the department

Senior position

In addition to the primary and secondary responsibilities, the Senior Executive Assistant is expected to assume leading roles in these duties, e.g. lead in the coordination of events, as well as to train and mentor junior Executive Assistants where necessary.

SECONDARY RESPONSIBILITIES

• Assist in the maintenance of the department’s generic email account
• Assist in the maintenance of the department’s webpage on the intranet
• Perform any other duties as assigned by supervisors and/ or Director, CRU

Principal Reaponsibilities

• Work closely with the general practitioners (GP) in taking consent from the patients for enrolment, referral and care coordination process within the primary care network;
• Extract the clinical data (patient demographics and parameters) from the various clinical management systems (CMS) and enter these information in the chronic disease registry (CDR);
• Work closely with the nurse in identifying patients who require nurse counselling;
• Prepare and submit the patient appointment list to the GPs for vetting;
• Liaise with the service providers in scheduling ancillary service deployments;
• Schedule patients’ appointments for ancillary services, calling of patients for appointment reminders and collecting of payments during deployments;
• Track and submit monthly ancillary reports;
• Prepare the reconciliation reports after each deployment;
• Track the defaulters and to liaise with the Clinic Assistants to ensure patients adhere to their scheduled chronic visits, blood tests and screenings;
• Check through the CDR and highlight to the GPs the patients who have missing clinical indicators minimally twice a year.

Secondary Responsibilities

• Support the admin team in administering surveys and collating data for analysis and reporting;
• Provide support in gathering feedback on ground issues;
• Perform any other duties assigned by the Reporting Officer.

Requirements

• At least Diploma with experience in interacting with people from diverse background, preferably with frontline service experience.
• Relevant work experience in healthcare or social/community services industry and coordinating care across the various care settings would be an advantage.
• Proficient using MS Excel (basic data entry and monitoring).

Responsibilities:

A Financial Counsellor is required to report to both Finance department and Operations department. Responsible for the counseling of patients on the use of CPF (Central Provident Fund) Medisave accounts, explain chronic care management packages available at our polyclinics, process case documents and work with polyclinic staff to provide a wonderful visit experience for patients. A Financial Counsellor is expected to perform all of the following areas of financial counseling functions:

• Counsel and explain to patients the principle and ruling of MediCare Assistance scheme.
• Assess patients' financial status and eligibility.
• Assess and perform medical bills waiver for eligible patients.
• Counsel patients on managing their medical bills settlement during assessment.
• Attend to patients' enquiries and assist in the application for medical subsidy & financial assistance schemes.
• Ensure the completeness of application submitted.
• Ensure proper filing and safekeeping of Medisave Authorization Form (MAF).
• Perform reconciliation of MAF (Medisave Application Form) against Medisave sign-up in the system on daily basis.
• Conduct peer's audit check on MediCare Assistance.
• Perform checks on documents submitted to Finance department for patient refund voucher, waiver, etc.
• Operate as Business Office for clinic by acting as the main contact for Finance department on finance-related matters.
• Provide support to clinic staff as & when requested.
• Any other responsibilities as assigned by the supervisors.

Requirements:

• Diploma holders in Accounting / Business / Commerce / Economics. with at least 2 years of working experience. 1 to 2 years working experience in customer service, healthcare setting or social/community services will be an added advantage..
• Good proficiency in MS Office applications such as MS Excel.
• Strong organisational skills, meticulous and able to work independently in a fast-paced environment.
• Good interpersonal skills to relate to people.
• On 5.25 workdays, alternate half Saturday.
• To base at Polyclinics located at Woodlands, Sembawang, Yishun, Khatib, Ang Mo Kio, Hougang, Toa Payoh, Geylang & Kallang.

What you’ll do as a Psychologist!

You will conduct consultations within a multidisciplinary team, for patients with chronic illness who have stress-related mental health problems to enable them to better manage their medical conditions. You will design and conduct relevant workshops/group sessions for patients and the public and be involved in research to advance the understanding of practice in primary care. In addition, you will also design programmes and conduct training for clinical staff in NHG Polyclinics.
