Job Description:
Summary of Responsibility
In keeping with the Mission/Vision/Values of Centerstone, a Tele-Health Counselor provides individual counseling to promote personal growth and resiliency in the form of emotional, cognitive, behavioral and/or psychological changes in clients with a primary mental health diagnosis and/or substance use diagnoses. This includes performing continuing assessment of the client’s needs and functioning, maintenance of appropriate documentation and development and implementation of personal growth plans consistent with the client’s therapeutic needs. A Tele-Health Counselor is required to use independent discretion and judgment when providing services to clients. As part of Centerstone’s team-oriented philosophy, a Tele-Health Counselor is expected to actively participate in variety of cross-functional teams and performance improvement initiatives.
ESSENTIAL DUTIES
Completes job requirements with independent action and resourcefulness and seeks out additional duties during periods of downtime. Self-starter, organizes time, effort, and use of resources to achieve desired outcomes.
Follow all Work At Home guidelines established by program and program supervisor, including having a professional and confidential home office space to conduct counseling.

Treatment must be recovery focused taking into account trauma informed care practices. Provides consistent, personal growth-focused services to all assigned clients by maximizing time spent working directly with clients to assist their personal growth process. To demonstrate clients are the first priority, ensure hours of care expectations are met and time is dedicated to providing personal growth services to clients, their families and collateral contacts.

Provides outpatient counseling services utilizing evidence-based methodologies to provide excellent clinical services. Prepare for and conduct individual and family counseling for assigned caseload.

Assist service recipient in developing an individualized treatment plan, must be person centered, family centered, and service centered. Monitor service recipient progress in treatment and participate in treatment plan reviews as assigned. Update treatment plans as appropriate. Submit treatment plans at all stages for medical review, if applicable.
Conduct intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned. Conduct assessment and write assessment summaries, as assigned.
Assist service recipients with making linkage with internal and external resources as outlined in treatment plan and in coordination with integrated healthcare services. Maintain appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate.
Assist service recipient in making discharge plans and provide follow up services as assigned.
Demonstrates an ability to manage crisis situations, including danger to self and others, to ensure the safety and stabilization of clients and to make appropriate referrals/linkages when clinically indicated.
Attend staff meetings, clinical staffings, and other internal meetings as assigned. Participate on committees as assigned, including quality assurance functions. Maintain appropriate communication with other employees and work as an effective member of treatment team.
Meet and maintain applicable certification and licensure requirements.
OTHER DUTIES AND RESPONSIBILITIES
Participate in training designed to enhance job-related skills. Provide community education as assigned. Perform other duties, not inconsistent with qualifications and regular duties, as assigned.

- Knowledge and skills in Trauma Informed Care (TIC)

- Knowledge of inpatient treatment resources, including appropriate admission procedures

- Knowledge of applicable regulatory guidelines and codes, agency policies and procedures, and federal and state laws.

RESPONSIBILITY
Supervisory Controls: The Manager along with the Director assigns work priorities, provides ongoing clinical supervision, and interprets and enforces policies and procedures for the Tele-Health Counselor as necessary. Guidelines: Board, Administrative and program policies, licensure, accreditation, statutory and regulatory standards restrict the Tele-Health Counselor’s discretion on treatment assessment, services, and record-keeping. A Tele-Health Counselor has the prerogative to propose changes in practice, procedures and policies that affect the Clinical team.
DIFFICULTY
Complexity: Position is characterized by a series of complex tasks. Although routine is established setting certain tasks at certain times, the extreme variation in service recipient needs requires constant adaptation. Emergency situations occasionally disrupt routine operations and may require more complex responses.
B. Scope & Effect: This job affects the physical, emotional, and psychological well being of service recipients. The agency depends on the Tele-Health Counselor to appropriately provide clinical services which meet requirements set forth in external grants/contracts and meet requirements for third party reimbursement.
PERSONAL RELATIONSHIPS
A. Personal Contacts: Tele-Health Counselor has daily contact with service recipients, collaterals, and agency staff; regular contact with external referral sources, other service providers and advocates; and may have contact with general public.
Purpose of Contacts: Purpose of contact with service recipients is to provide clinical services, assessment, non-violent crisis intervention, and brief counseling as needed, as well as consultation regarding these services. The purpose of contact with others is primarily to gather and exchange information.
ENVIRONMENTAL DEMANDS
Physical Requirements: The work is primarily sedentary, but requires skill in non-violent crisis intervention. Must be able to lift up to 25 lbs.
B. Work Environment: Work is primarily performed in a remote setting, including in a home office, in an office environment or community setting such as a hospital, police department, county jail, school, service recipient home, etc. Must be available to have a flexible schedule, including morning and evening work times.
MINIMUM CREDENTIALS/REQUIREMENTS
Requires a Master’s Degree in Human Services and Licensure or CADC within two years of hire. Must have valid driver’s license in applicable programs. Providers utilizing electronic health records and/or other automated/electronic documentation systems are required to have computer and keyboarding experience and skills or the ability to develop such skills within required timeframes in order to meet the level of proficiency set by the agency. Direct Service Providers utilizing electronic/automated documentation systems may be required to undergo a computer and keyboarding assessment test to determine proficiency.
A Master’s level Tele-Health Counselor must meet Qualified Mental Health Professional status in relevant programs. All employees in the Tele-Health Counselor position must successfully complete required background checks and pre-employment screening.
