The post holder is an experienced specialist Health Care Professions Council Registered Biomedical Scientist who provides a continuous and comprehensive blood science service as an aid to effective diagnosis and treatment. The post entails confirmation of the analytical and technical validity of investigations, liaising with clinical and blood science staff to ensure diagnostically abnormal results are urgently acted upon.Where undertaken, staff committing to working unsocial hours will have significantly different working patterns and responsibilities to routine core hours. This is because these staff operate and communicate as a lone front line self-governing service provider and assume first line responsibility. MAIN DUTIES:o Provide a rapid, reliable and efficient service by performing a comprehensive range of routine and specialized analyses. o Perform investigations and analytical techniques unsupervised whilst organizing own work tasks, normally under the direction of a Section Leader. o The post holder is required to rotate through all the sections of the department including specimen reception. o Apply the approved Standard Operating Procedures used in the Department. o Assist in the development of Standard Operating Procedures, and recommend improvements and amendments as necessary. o Perform complex manual and automated analytical investigations requiring specialist skills. o Operate complex, high value analytical equipment without supervision. o Maintain, troubleshoot and repair faults in complex laboratory instruments, ensuring essential services are maintained in the event of technical failure. o Prepare reagents, control materials and calibrators with a high degree of accuracy and precision. o Required to work with hazardous and noxious chemicals, and with frequent exposure to uncontained body substances. o Check reliability of results to ensure accuracy and precision by applying quality assurance statistical criteria. o Interpretation of results requiring subjective judgement and analytical expertise. o Recognise grossly abnormal and life threatening results. o Validate and report test results in a timely fashion so that they may be acted on as necessary by clinicians. o Provide technical interpretation and comment on validity of results. o Maintain accurate and detailed records of patient results, quality control data and calculations. o Participate in departmental audit and internal / external Quality Assurance programs. o Work with laboratory and Trust information systems. o Maintain, troubleshoot and repair faults with laboratory point of care equipment in clinical areas and liaise with service engineers and ward staff as required. o Supervision of MLA and Trainee BMS staff. o Assist in the departments stock control procedures. o Adhere to the departmental Quality management systems. o General clerical and administrative tasks. o Selection, matching and timely issuing of correct blood products in response to medical prescription, o Maintain adequate supplies of all blood products, liaising with Blood Transfusion Service and other outside agencies. o Maintain an accurate audit trail of the fate of all blood products. o Ensure that blood products are stored in compliance with NationalGuidelines

Identify patients approaching the end of life and initiate fast track processes to preferred place of care in accordance with The National Framework for NHS CHC. Support and respond to the Trust Escalation Processes, as guided by the Head of Complex Case Management /Head of Clinical Flow and maintain close liaison with Bed Management. Represent the Complex Case Team at bed meetings communicating appropriate and accurate information. Contribute to the maintenance of processes which support The Community Care (Delayed Discharge) Act 2003 and ELHT Patient Discharge Policy. Ensure compliance with other related Trust Policies and Department of Health Legislation with regard to discharge planning processes. Support the Head of Complex Case Management to implement strategies for ward staff and professionals to determine realistic discharge dates for timely and effective discharge planning patient flow. Screen referrals made to the Central Point of Referral in Complex Case Management, signposting to alternative pathways as appropriate to meet identified needs. Screening and tracking of referrals, monitoring assessment and transfer timescales. Maintain databases as required. Maintain accurate record keeping at all times.

To promote and maintain effective communication channels between all Health and Social Care departments/agencies in the acute and community settings. Proactively advise and support staff to initiate the early discharge planning of patients with vulnerable/complex needs ensuring appropriate assessments are completed in a timely manner to facilitate discharge. Identify, progress chase and monitor delayed discharges and lost bed days within the patient journey and referrals. Escalate delays and concerns to the service leads. Operate the Continuing Health Care desk/ processes set up with the Intergrated discharge service, working on own initiative to ensure discharge planning process commences as soon as possible after admission and that immediate discharge is achieved once the patients is medically optimised. Contribute to and lead on daily, weekly, and monthly reporting related to the area of work. Prioritise own workload to ensure deadlines are met and a quality, responsive service is provided. Identify and discuss any potential discharge problems/delays with ward staff and patient/relatives to resolve issues as soon as possible. This will be done on a face-to-face basis. Visit wards as appropriate to support the education of complex discharge planning and specific specialised care coordination that is available to support discharge for complex and highly complex patients. Alert the Integrated Discharge Service Lead/Head of Complex Case Management of any unresolved issues/conflict/barriers preventing resolution. Maintain accurate records and participate in internal or Department of Health audits as required in relation to the discharge planning process. Act in accordance with Code of Professional Conduct and ensure current registration is maintained. To provide pro-active and responsive support to the integrated discharge team members. Establish and maintain positive and productive joint working with system partners and staff at ward level. Based within the Integrated Discharge service Team on the Burnley General Hospital site however the role involves working across all ELHT sites to meet the needs of the service. The post holder will be working independently on their own initiative without direct supervision when out of the office base, reporting back progress and informing the complex case managers of any problems arising. Ensure patients case notes and referral information are handled according to Trust policy and recorded appropriately. The post holder will be required to maintain a confidential and professional approach at all times. The role will involve communicating with patients, relatives, colleagues, nursing and AHP professionals across health and social care, either by telephone or face to face. Participate and correlate information, with regard to the delayed discharge process, from reports/overviews or ward visits and communicating with ward co-ordinators which will initiate alerts, to enable qualified members of the Integrated Discharge service Team to action resolution of delays in the discharge process. Give advice to ward staff at all levels as queries arise with regard to Discharge within remit of knowledge and role. Escalate unresolved issues, disputes and delays in the care process to the senior complex case manager to enable these to be resolved in a timely manner. Prioritise and organise own caseload of work, visiting wards on a daily basis, working on own initiative to ensure deadlines are met and a high standard of service is provided. Support discharge processes that comply with The Hospital and Community support policy operating model 2021. Refer any complaints or unresolved issues to the line manager or delegated person. Respond to bleep/calls in a timely and professional manner. Screen and input information on referrals and trusted assessment documents. Identify risk triggers within the referral/trusted assessment documents and initiate appropriate action. Work within limits of own competence and responsibility, referring issues arising beyond these limits to qualified team members for action. The post holder will be expected to utilise PAS, analyse and input to electronic central referral logs, electronic patient tracking systems and live client data bases within Social Services. Provide additional support to the Integrated Discharge service, as necessary, or to implement improvements, training and provide cover for annual leave and sickness. Understand own limitations and areas of responsibility. Document accurate records of work undertaken, ensuring variation of expected outcomes are highlighted. Collate and maintain monthly statistics as required.
