We are currently seeking an enthusiastic and motivated Physiotherapist to join the Therapy Assisted Discharge service (TADS) at Maidstone Hospital.

TADS is a team of occupational therapists, physiotherapists and therapy assistants that enable early discharge from hospital by providing access to therapy assessment and rehabilitation at home within 24 hours of discharge from hospital. The team is part of West Kent Home First and works in collaboration with community health, social care and voluntary agencies.

TADS operates from 8am to 6pm seven days a week. The post therefore involves joining a seven day roster with opportunity for flexible working hours and enhanced rates of pay. Relocation expenses may be payable to the successful candidate providing eligibility criteria is met. This will be discussed with short-listed candidates during their interview.

We are looking for someone with a commitment to multi-disciplinary working, excellent organisation and communication skills and a proactive approach to personal development. This is an ideal opportunity for an experienced Band 5 or Band 6 wishing to develop clinical skills in intermediate care within an established and supportive team.

Full Job Description

• To undertake the full range of offender management tasks with offenders assessed as low or medium risk of harm and to support the Probation Officer grade in high risk cases.
• When providing case manager support, to contribute to the delivery of the Risk Management plan and report significant changes relating to risk of harm and/or of reoffending or any non-compliance within agreed enforcement procedures
• To use computer based systems to produce, update and maintain records and other documentation within agreed timescales
• Ensure effective referrals to services and facilities and communicate with offender management staff, interventions staff, service providers and external agencies to review progress and associated risks.
• To undertake prison, home or alternate location visits as required in accordance with service procedures and policies.
• To undertake work in the court setting, including the completion of appropriate reports on cases and prosecution of breaches.

• Experience of working and supporting a diverse range of people who have experienced a range of social/personal difficulties, including working with groups and individuals to motivate and change behaviour.

We have an exciting opportunity to recruit an NMC Registered Nurse Associate to join the existing team based at Maidstone. This is a seven-day service, working between 08:00 and 16:00, with a late shift of 10:00 – 18:00 on a rotational basis.

The team support patients from GP practices within the Maidstone Central Primary Care Network (PCN).

The teams provide nursing care to predominantly housebound patients in the Maidstone town centre area. Each team takes responsibility for a defined geographical patch, which allows good working relationships with local GPs, care homes, Social Services, voluntary organisations and other stakeholders.

The District Nursing Service also provides local clinics for patients requiring their clinical skills. We believe the addition of the new Nursing Associate role will complement the current skill-mix for providing nursing care within a community setting.

This is a highly rewarding role, and nurses are supported to achieve all the clinical skills required for the role.

Clinical areas will include wound management care, end of life care, medicines administrations and bladder and bowel care, amongst other duties.

You will be supported by senior staff and the team; therefore, these posts will suit those wanting to move to a community setting. Staff will be required to drive their own car and are able to claim mileage costs and access the NHS car lease provision, following their probationary period.

An exciting opportunity has arisen for a dietitian to develop and enhance their knowledge and practice in providing nutrition for acute patients at Maidstone Hospital.

The successful applicant will join the friendly dietetic team at the Maidstone and Tunbridge Wells (MTW) NHS Trust and support the Acute Team Lead at Maidstone. Working with members of the MDT the team delivers a dietetic service to acute and critical care inpatients.

You will have the support of senior dietitians and have the opportunity to develop and enhance your dietetic skills in a variety of specialties including critical care and ITU, respiratory and gastroenterology. ABOUT THE ROLEAs a Bank Admin Assistant at a Barchester care home, you'll carry out a range of responsibilities to help us deliver the quality care and support our residents deserve. In this varied role, we'll need you to answer the phone, handle our files and support our managers. You'll also meet and greet visitors, engage with residents and show prospective clients and their families around so they can see what makes us so different.
