The Associate Chief of Staff, Geriatrics and Extended Care (ACOS/GEC) is responsible for providing a full range of geriatric services for a diverse group of aging Veterans within the Hines VA Hospital. These duties include but are not limited to referral consultation covering all issues of Geriatrics. The ACOS/GEC will perform as a consultant to our inpatient service, Emergency Medicine service, and others as appropriate. In addition, the ACOS/GEC will perform Geriatric Evaluation and Management and will develop treatment plans for our residents in the Community Living Center. In addition to excellent leadership skills, the ACOS/GEC will be knowledgeable and proficient with VA Performance Measures and the principles of System Redesign, demonstrate leadership qualities, as well as strategic planning skills, dedication to patient-centered care, and a commitment to organizational stewardship. ACOS/GEC is required to lead, give direction, provide knowledge, skills and abilities in the area of geriatric and rehabilitative, and lead professional staff in these clinical domains.The Associate Chief of Staff, Geriatrics and Extended Care is responsible to the Chief of Staff. Leadership assigns patient care management by defining objectives, priorities and deadlines; general guidance is provided, but functions and responsibilities are carried out independently. Duties and Responsibilities of the Associate Chief of Staff, Geriatrics and Extended Care (ACOS GEC) will include but are not limited to

• Ensures clinical care is delivered in accordance with hospital and VHA policy, meeting or exceeding community standards and VHA national benchmarks.

• Provides direct clinical and administrative oversight for medical students and Internal Medicine residents, and fellows from Loyola University Medical Center (LUMC).
• Develops performance metrics for all provider classification to drive performance and to direct desired behavior.
• Is in compliance with VHA Scheduling Directives to enhance the management of access.
• Is actively engaged in facility Quality Improvement activities, including compliance with the VA credentialing/privileging process and ensuring that an active Peer Review process is in place for Geriatrics and Extended Care Service providers.
• Actively engaged in Service Recovering and have knowledge of SHEP and other patient satisfaction data sources. Uses customer service data for service improvement.

FUNCTIONS OR SCOPES OF PRACTICE:

Knowledgeable in United States Pharmacopeia (USP) 797/800/795, Pharmacy Law, VHA Directives, and Drug Enforcement Agency (DEA) regulations as they relate to practice within the VA Healthcare System and its organizational elements.

Using aseptic technique, compounds both complicated and uncomplicated parenteral admixtures in the form of large volume, small volume, infusion syringe or another modality as required.

Prepares sterile ophthalmic preparations and other complex sterile pharmaceuticals such as total parenteral nutrition and cardioplegia solutions.

Correctly interprets orders, selects, and measures appropriate ingredients, mixes ingredients in an aseptic and effective manner, inspects the final product for accuracy, suitability, and elegance, labels the product, maintains records of the manufacturing process, and supplies the product for delivery.

Understands specific handling considerations for storage, distribution, and disposal of medications.

Operates and maintains complicated pharmacy equipment including but not limited to Omnicell TM /Pyxis TM, laminar flow hoods, total parental nutrition (TPN) compounder, repeater pumps, etc.)

Ensures that the IV cleanrooms are cleaned daily, and appropriate logs are completed for daily, weekly, and monthly cleaning.

Stocking and maintaining the Pharmacy Service preparation area(s), reviewing anticipated inventory needs and notifying the procurement technician in a timely manner of products required.

Participates in glove fingertip and media fill testing, maintain competences passing didactic practical skill assessment initially and annually thereafter.

Assists in airborne particulate and microbial testing and surface testing to ensure cleaning schedules are met.

Responsibility for keeping abreast of changes in the Pharmacy Service (i.e., interdepartmental memoranda, relevant station memoranda, and policies and procedures.

Assists in training and orientation of new staff to ensure consistency in the training process.

Other Duties:

Performs daily refrigerator and or freezer temperature checks and/or monthly inspections for outdates according to local policy and standard operating procedures.

Edward Hines Jr. VA Hospital is seeking a full time board certified or board eligible Neurologist qualified to diagnose, treat, and manage patients with neurological diseases, according to standard, usual, and acceptable methods and techniques. Applicant with diverse general Neurology experience, with cognitive or multiple sclerosis experience preferred but not limited to those specialties. Duties also include outpatient and inpatient neurological evaluations; patient examinations; diagnosis; consultations and inpatient service. ACLS /BLS required. Patient Care Duties at Edward Hines, Jr. VA Medical Center

• Develop and maintain a faculty outpatient Neurology practice and academic program, that provides extensive expertise in the treatment of various neurologic conditions
• Neurology Inpatient Service:
  • Evaluates and manages the neurology inpatient service as the attending physician and advises the primary physicians on inpatients hospitalized at the Edward Hines, Jr. VA for which consults from the Neurology Service are requested. The Assistant Nurse Manager (ANM) supports the Nurse Manager (NM) with supervisory duties and is responsible and accountable for the daily operations of the Home & Community Based Care Programs (Home Care). The ANM provides coverage for the NM as needed and is responsible for the direction and supervision of specific orientation, training, and development of staff. • Assists cooperators in understanding of and compliance with the Migratory Bird Treaty Act (MBTA), Endangered Species Act, Bald and Golden Eagle Protection Act, and other wildlife-related laws.
• May be required to take extensive training and recurrent training on wildlife damage or disease recognition, control methods, and techniques
• Communicates safe and effective damage management techniques through outreach and other channels to the general public.
• Assists other WS employees as requested and may assist in training new WS personnel.
• Modifies or adapts established damage or disease control techniques as necessary to meet local conditions and address specific environmental, economic, or political considerations.
• Recommends, to supervisor, areas for potential research or developmental study, to reduce the risks of damage to resources or disease transmission to domestic animals and humans.
• May develop and deliver training for cooperators, stakeholders, partner agencies and organizations, or other entities.
• Establishes and maintains cooperative relationships with other Federal, State, county and city government agencies, Tribes, corporations, educational or research institutions.
• Gathers, compiles, evaluates, and presents data on damage or hazards caused by various wildlife species to cooperators, stakeholders, and colleagues through project or program reports .

Job Summary

This position will be Remote / Telecommute

• Work Schedule - Monday through Friday (9:30 am to 6:00 pm)
   

• Required - Certified Diabetes Care and Education Specialist (CDCES)  Certification

JOB REQUIREMENTS:

• Current, unrestricted license in the state of operations in one of the following: Registered Nurse (RN), Licensed Professional Counselor (LPC), Registered Dietitian (RD) or Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Exercise Physiologist.

• 3 years of clinical experience.

• 1 + years’ experience in case management or condition management in a managed care/health insurance company

• Chronic Care Professional (CCP) certification or Certified Case Manager (CCM) (where contractually obligated)

• Patient education experience.

• Computer/system software experience including Web.

• Verbal and written communication skills; analytical skills; sound clinical judgment.

PREFERRED JOB REQUIREMENTS:
