Information about the Role:
Working as a RMN working at Sturdee Community Hospital, you will:

• Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. Administer nursing care to ill, injured, and long term service users.
• Behave in a professional and discreet manner at all times and in line with the Nursing and Midwifery Council (NMC) Code.
• Be competent and confident in managing people with long term health conditions.

Full Time / 37 Hours Per Week / 18 Months Fixed Term




A unique opportunity has arisen for a qualified occupational therapy to join our Home First Service. The Home First Service is made up of our co-located services from across Adult Social Care and Community Health (Nursing / Therapy Teams). The service enables timely hospital discharges for those needing care and support by avoiding the risk of de-conditioning whilst optimising their independence. The Home First OT will work closely with the Reablement Service which supports the majority of hospital discharges and the Integrated Crisis Response Service which supports hospital avoidance and will ensure our intervention is strengths based and person centred. The OT within this role will work collaboratively within the co-located multi-disciplinary team ensuring people have the opportunity and support to maintain and maximise their independence. A key area of work will also include supporting people with double-handed care needs with a view of assessing their overall level of need before long term care is commissioned.




This role will be based at the Neville Centre on the Leicester General hospital site, with your own designated car parking working mainly core office hours 8:30am to 5:00pm Monday to Thursday and 8:30am to 4:30pm Friday. However, there will be flexibility and autonomy within this role to help manage weekly commitments and help provide support out-of-hours and weekends too.



What you’ll be doing




As an OT within the Home First Service, you will be completing assessments within the person’s home environment, with intervention based around their needs, what is important to them and goals they wish to achieve. You will be required to complete comprehensive moving and handling assessments, assess, and prescribe appropriate equipment and recommend adaptations. You will be expected to work collaboratively with the Community Health Therapy Team, Reablement/ICRS Service, and the Reablement Social Work Team, ensuring effective communication and an integrated and co-ordinated approach at all times.

As a Qualified Occupational Therapist who is registered with HCPC you’ll be committed to a strengths-based approach and positive risk taking. You’ll have excellent interpersonal skills, meaning you can work well alongside people in complex situations and circumstances. As a valued member of a busy Home First Service, you’ll have good time management skills and be able to prioritise between competing demands.




Due to the nature of this role, you’ll be subject to an Enhanced Adults and Children’s DBS. In addition, a Driving License is preferred however, if this isn’t something you have, you must be willing and able to travel to visit people in their homes across the city.

This post will have a key role in the delivery of the Integrated Community Equipment Loan Service ICELS for Leicester, Leicestershire and Rutland. The ICELS plays a critical role in supporting health and social care services across LLR delivering care and support to the people of LLR including hospital discharge, crisis response to avoid hospital admission, end of life and palliative care, support to keep people independent and remain in their own homes. This is a very busy service supporting the wider health and social care economy across LLR. The service deals with in excess of 120,000 pieces of equipment to around 30,000 people each year. The delivery of the ICELS is contracted out to an external provider Medequip.




Working as part of the wider ICELS Team who provide contract performance and quality assurance monitoring of the service provider, day to day operational management of the service, support for health and social care professionals and a clinical team consisting of an Occupational therapist, Physiotherapist, Lead Nurse, Triage Nurses and Technical Instructors who undertake active recall of equipment in the community, triage of orders placed by health and social care professionals.




The ICELS has an office at the Equipment Provider’s base in Glen Parva. The team works both remotely and at the office. The role will work Monday to Friday between 8.30 a.m. to 5.00 p.m. 4.30 p.m. on Friday, flexible hours and working arrangements are available. Attendance at the office will be required for meetings, training and other matters that require face to face contacts.




What you’ll be doing




Triaging equipment orders placed by health and social care prescribers. Providing support, advice and guidance to health and social care professionals working in organisations/services across LLR. Working closely with other members of the ICELS team to deliver operational efficiencies and budget savings. Providing daily operational support to the equipment provider to ensure effective and timely delivery of equipment.




What you’ll need




A Diploma/Degree from College of Occupational Therapy and registration with the Health and Care Professions Council HCPC and responsibility for maintaining registration.

Extensive experience of working in an occupational therapy role in NHS Community Health Services or a Social Services Department. As well as experience of multi-agency or multi-disciplinary work or hospital discharge processes.

Up to date knowledge of and experience of assessing for equipment to meet needs of a range of people to aid their daily lives including specialist disability equipment, both new and recycled back onto shelf for reuse.

Maintain clinical experience and provide expertise in the specialist area of Paediatric Epilepsy care to patients, families and professionals. Liaise with Leicestershire Education and Effectiveness Partnership (LEEP 1) to coordinate training for schools in CYP epilepsy including use of emergency medications and support development of epilepsy champions in schools. To provide a nurse-led review clinic to offer additional support to families/carers and consultant team. To lead the development of a working partnership with the Learning disability and Autism (LDA) collaborative of LLR ICB and plan for 6 joint clinics (alternate months) per year with specialist LD nurses. In collaboration with paediatric education leads, develop dedicated training specific to the Network and ensure a development training programme is in place for nurses and other professionals who may care for children requiring Paediatric Epilepsy Services. Develop dedicated guidance, SOPs and protocols in the Paediatric Epilepsy Service as relevant to the development of the Network. In conjunction with the wider Epilepsy Nurse Specialist team, work to ensure that transitions in care, for example, between childrens and adult services, are as seamless as possible and that information to support the experience of the whole transition process is accurate and readily available. Provide support to patients, their families and professionals around end of life care and unexpected death, working closely with the childrens and adult palliative care services. In particular, work to ensure a consistency of approach and management of expectations for this sometimes very unwell patient group. Work with parent/patient representatives and other health personnel to improve patients and families experience throughout their journey. Provide advice and clinical support during investigations into specific patient complaints where appropriate. Independently prescribe clinically relevant medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence- based practice, national and practice protocols and within scope of practice to optimise treatment.
