The post holders will manage a caseload of clients with mental health conditions including dementia, acting as lead professional, providing nursing assessments and interventions on an individual and group basis. The Memory Service forms an integral part of the Community Mental Health Team, providing a range of interventions for people living with mental health conditions. North Liverpool PCN is looking for a highly organised, motivated individual to support the day-to-day running of our Womens Health Service and Supporting of the PCN. The post holder would need to be extremely efficient, professional & understand the importance of good access to high quality womens health care (especially in the current climate). We would expect an applicant to be very good at working in a team and have a passion to help develop our services. The post holder is expected to be a strong communicator and have excellent administrative skills. The successful candidate is expected to have an enthusiasm to train in new skills and to work alongside our GP lead for Womens Health to help develop and expand the Womens Health Hub and to also support the primary care network Care co-ordinator. The successful candidate will work 3 days as a womens health care coordinator and 2 days supporting the PCN.

The post holder is responsible for the delivery of clinical care and support under the supervision of a registered nurse to a defined population. Supported by a nurse practitioner, they will deliver care within the boundaries of their role. They will work effectively and collaboratively within the team to meet the needs of patients, support the delivery of policy and procedures and provide clinical leadership. Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes

Design and deliver and evaluate non-accredited health and wellbeing courses as support health activation. Deliver and evaluate accredited health and wellbeing courses to support health activation. Follow all learning operational processes to ensure accurate monitoring and recording of service activity including enrolment, attendance, outcome measures, service user feedback, curriculum governance and content development. Contribute towards the provision of a comprehensive curriculum utilising 80% of post holder capacity in face-to-face delivery. To travel and facilitate in a range of Life Rooms and peripatetic sites across a designated footprint. Contribute to the maintenance of ongoing registration with accredited providers. Awareness of and commitment to evidence-based practice to inform design and delivery. This may include educational theory, local and national initiatives and health and wellbeing priorities. Ensure a fully differentiated curriculum and resources are available and delivered to enable participation and learning for people with a range of additional and/or specialist needs. Be responsible for the application of differentiation methods to ensure an equitable learning offer that is inclusive of a range of learning needs and styles. Ensure culturally appropriate and diverse materials and tailored delivery that are accessible to a range of communities. To provide oversight of service journey through a stepped approach to learning to ensure service users are accessing a suitable learning level to meet their needs. Maintain a focus to support service users to increase independence in engaging with community assets through establishing transition pathways and self management approaches. Have an awareness of community assets and support service users to access appropriately, liaising with partners where necessary to facilitate engagement or transition. Collect service activity data and outcome measures that contribute towards evidencing the impact of interventions. Collate and present service activity data and outcome measures that contribute towards evaluation, quality and performance processes. To monitor, promote and sustain service user engagement through proactive communications and positive relationship building to support attendance and completion rates. To participate in Quality Improvement projects under the guidance of The Learning Delivery Manager and Quality Improvement Partner. To follow and uphold governance procedures within the service. Facilitate co-production processes within the activation through learning model to ensure the curriculum is based on identified, emerging and informed need. To provide appropriate support to individuals experiencing mental and emotional distress or presenting with challenging behaviour, whilst maintaining professional boundaries. To follow and implement safeguarding policy where required, reporting incidents through the Trusts Incident Reporting system. To support staff within line management structure when dealing with a safeguarding concern or issue. To provide leadership to staff within the line management structure. To adhere to line management and supervision process including HR and staff development. To support with recruitment processes within the learning team.

Undertaking person centred lifestyle assessments, in a sensitive manner, empowering mothers to make positive change. This may include discussions regarding nutrition, exercise, smoking, alcohol use, substance misuse, sexual health both in pregnancy and in the postnatal period. To provide appropriate information/education to service users, families and carers relating to physical health and lifestyle ensuring informed decision making. Make appropriate referrals and support to access relevant lifestyle improvement initiatives available. Provide support, motivation and development of appropriate action plan for each client. Carry out review meetings to assess progress and provide long term motivation and guidance, using solution focussed or motivational interviewing skills as appropriate. Produce progress or activity reports as required in relation to interventions offered. Evaluate and feedback clients responses to the intervention and modify in discussion with multi disciplinary colleagues. Whilst working with clients and community groups, identify barriers and possible solutions to making sustainable lifestyle changes, reporting these back to the lead professional involved. To contribute to planning, development, implementation and evaluation of individual care interventions related to physical health and wellbeing during the perinatal period. To identify, obtain and if appropriate carry out appropriate diagnostic/ assessment techniques to identify and respond to clients physical health needs in a range of settings. For women who have severe mental illness and are prescribed mood stabilisers or antipsychotic medication work alongside lead professionals to ensure that appropriate monitoring tests are performed and accurately recorded, and any concerns escalated as necessary. If necessary and appropriate, accompany and provide support to women undergoing physical health tests. Obtain necessary results from blood tests and other investigations and record accurately in electronic patient record, escalating to medical staff when necessary. Liaison as necessary with maternity and primary care regarding the physical health needs of clients.

The post holder(s) will be part of a multi-disciplinary team, contributing to the effective provision of Perinatal and Infant Mental Health Services to an identified caseload of women.

Improving the physical health of our clients and reducing the impact of inequality on poor health outcomes (both maternal and child) has been identified as a need in our current service provision and this post reflects this.

The post holder will assist in the identification of physical health and lifestyle needs, and participate in the delivery and evaluation of interventions developed to meet those needs. Working alongside mothers experiencing moderate to severe perinatal mental health problems, and including those with pre-existing severe mental health disorders. The post holder will contribute to holistic community assessments, in a range of settings, with a strong emphasis on promoting optimum physical health and wellbeing. The post holder will actively work with families and carers in their own home and in the community settings flexibly and will offer practical support and advice based on the identified needs of service users , partners and their children.
