We welcome applications from committed and enthusiastic Nurses with extensive experience within the Learning Disability field who are passionate about working as an integral part of our established and supportive Community Learning Disability Team, based in East Cheshire. The team comprises of nurses, speech and language therapists, occupational therapists, physiotherapists, psychologists and psychiatry. The successful candidate will work autonomously, managing a defined case load of adult service users with Learning Disabilities and complex physical health needs.

The successful candidate will be expected to work across a variety of settings and perform nursing functions relating to health facilitation and liaising with hospitals when this is required and support the wider Team in understanding the importance of health facilitation and the making of reasonable adjustments.

The post holder will work as a member of the specialist multi-disciplinary team, carrying out assessments and providing effective treatment, care and support to clients who may be experiencing physical health problems in addition to their learning disability.

The role will involve completing assessments and risk assessments that inform clinical interventions and contribute to the planning of interventions and the completion of care plans. The role will also involve being the Care-Coordinator for patients with complex physical health needs that will include the monitoring and reviewing of patients physical health needs to achieve desirable clinical outcomes. The Post Holder will work in an empowering way and promote healthy living so patients can take responsibility for managing some of their health needs when this is possible to do so.

All aspects of the role require effective communication and collaborative working with partner agencies, GP's to plan and deliver LD Awareness training and promote the importance of making reasonable adjustments to ensure patients receive the best possible care. The strong links with GP's will involve providing support to identify patients who have a learning Disability and to verify the accuracy of the LD Register. The Post holder will work closely with our Band 7 Health Facilitator who will offer on-going support to prioritise the community and health facilitation elements of this role.

The Post Holder will also work in line with the Dynamic Support Decision Tool that identifies individuals that are at risk of premature mortality and work with patients who are assessed as being Red or Amber.

£35,353 per annum (£18.08 per hour) – At the end of your preceptorship or for those with up to 2 years post-preceptorship experience

£40,241 per annum (£20.58 per hour) – For individuals with 2 years or more post-qualification experience

Weaver Lodge Treatment and Recovery Centre in Winsford, Cheshire, is a twenty

bedded Mental Health Rehabilitation Centre. Weaver Lodge specialises in the

rehabilitation of people with severe and enduring mental health conditions.

We aim to develop the skills to reintegrate successfully back into their communities

with the support of a strong MDT, including an occupational therapist, Responsible

Clinician and nursing team including support staff.

We pride ourselves on our community links and the support they give to ensure our

service users can meet their vocational and community network needs to support

them on discharge.

Weaver Lodge has up to a two year pathway that promotes discharge from day one.

We have a full assessment route for both the nursing and occupational therapist

team that is reviewed within the MDT and goals set in collaboration with the service

user on a monthly basis.

There are training opportunities for all staff including mental health awareness, care

certificate and health and safety as well as on the job coaching and other external

opportunities.

We offer regular supervisions and appraisals to ensure consistency and support is

offered to our team.

Weaver Lodge is looking for a Mental Health Nurse with the same high standards

evidence based practice as a standard delivery model.

The team act as a gatekeeper to admissions so the post holder will be able to consider referral information from other services and be able to contribute to decision making regarding the most appropriate treatment option. The post holder will also work in our triage hub, responsible for accepting and triaging urgent referrals from a variety of sources, for those experiencing a mental health crisis. You will be expected to maintain a high standard of electronic patient records. You will work collaboratively with colleagues in the team and across the community mental health service. There will be an expectation that they will work flexibly across the service which covers a 24-hour period, 7 days a week. We are looking for someone who is professional in how they communicate, work with care and compassion, are competent, committed, courageous, reliable, trustworthy and dependable in undertaking allocated tasks and responsibilities. The service has seen many positive developments in a short period, which we continue to play an important role in- supporting individuals in a Step- up House as an alternative to hospital, and having close working links with the Mindline phone and Crisis Safe Space services in the county.
