Bevan Place provides care to males aged 50+ with chronic and severe mental illness, cognitive impairment and physical health comorbidities using a recovery focused approach.

To assist the unit/clinical manager in leadership, managerial duties, role modelling and clinical leadership.

The Discharge to Assess and Intermediate Care at Home teams continually develop and enhance the seamless transfer of care from hospital discharge to usual residence. Home Treatment Team - Stockport (Heathfield House/SHH)

An opportunity has arisen, within the Home Treatment Team; based in the Community, for Senior Home Treatment Mental Health Practitioners (RMN /Social Worker/OT) to join our team.

The successful candidates will provide a key gatekeeping service for all inpatient admissions, will offer intensive home treatment as an alternative to inpatient care, facilitate early discharge/leave pathways to support discharge from inpatient services.

You will provide assessment, risk formulation/management and assessment for all service users.

In addition the candidate will provide Crisis interventions and also be able to plan and deliver care for all service users and provide supervision for junior colleagues.

You will actively participate in the development of the service and participate in the overall development of the service ensuring that the principles of Clinical Governance underpin all developments.

It is expected that you will deliver care in accordance with all Pennine Care NHS Foundation Trust policies and procedures, the Mental Health Act and community care legislation.

The successful candidate will be expected to work on a rotational basis over a 7 day off duty period.
