Advanced Specialist Mental Health Pharmacist – Crisis Resolution Home Treatment Team (Band 7/8a)

An exciting opportunity has arisen for an enthusiastic and self-motivated pharmacist with experience in mental health to provide specialist mental health services to our Crisis Resolution Home Treatment Team (CRHTT) in Berkshire West (2 days) and mental health inpatient wards (up to 3 days).

• To provide high quality, specialist clinical pharmacy service to the Crisis Resolution Home Treatment Team (CRHTT)and mental health inpatient wardsto ensure the safe, clinicallyeffectiveandcost-efficientuse of drugs

• Toparticipatein MDT meetings toprovideexpert medicines advice and support the wider medicines optimisation agenda

• To work closely with othercommunity-basedand inpatientclinical pharmacists to ensure seamless pharmaceutical care is provided throughout each patient’s journey

• To provide education and training to healthcare professionals, service users and carers

• Toprovideclinical pharmacy advice on the management of patients with mental illnessin order tomaximise benefit and minimise risk in relation to medicines

• Tobe responsible forupdating and reviewing relevant SOPs, guidelines,PGDsandePACTanalysis
