Knowledge, training and experience A qualified dental nurse should: Hold the NEBDN certificate in dental nursing/NVQ level 3 city & guilds in oral health care or recognized equivalent. (IRMER) Ensure that all dental instruments are decontaminated appropriately in accordance to the service policies/protocols and in a timely manner. Ensure that the surgery is decontaminated and cleaned and in a timely manner prior to the next patients treatment ensuring that the service policies and protocols are followed at all times. Ensure at the end of day that the surgery is decontaminated and cleaned following service policies and protocols. Ensure accuracy when recording and charting treatment plans within the R4 appointment system. Preparing and monitoring laboratory work and making necessary plans when work is not ready. Ensure that the service training needs are met by planning other commitments in order to attend. Making and recording appointments which suit the patient and the organisational requirements. Organising school screening when required and completing the returns prior to collation and analysis. Ensuring that equipment and building deficiencies and breakages are reported. Maintaining medical files in line with organisational requirements. Maintaining a regular supply of essential materials and ordering to replenish when necessary. Physical skills The post holder will be expected to: Move and handle patients from wheelchairs to the dental chair, either by hoist in line with Moving and Handling recommendations set out by the organisation. Walking to waiting area to receive and retrieve patients. On occasions work with limited scope for movement during treatments, whilst sitting for long periods. On occasions make repetitive movement during treatment. Be required to twist and bend whilst treating patients in order to maintain a good line of sight throughout treatment. On occasions lift pharmaceutical chests and deliveries of stock of whose weight will not exceed 4Kg. 6. Responsibility for patient / client care The post holder should: Ensure the patient is given the appropriate pharmacy advice leaflets, where applicable. Check the patient has understood how to take the medication they have been prescribed before they leave the clinic, notifying the dentist when a patient has not understood this information in order that this information can be re-iterated to them by the dentist. Dealing with treatment enquiries in the absence of a duty dentist. Ensure the patient is given the appropriate advice leaflet following dental extractions and the patient understands the importance of following the instructions on the leaflet. Observing the patients condition throughout treatment. 7. Responsibility for policy and service development implementation The post holder will be: Responsible for ensuring all policies, procedures and guidelines that are implemented within the service are adhered to within their own area of work. 8. Undertake assessments for all alcohol individuals who use our services as indicated in the Change Grow Live Alcohol Policy and Procedure, using the outcome to determine safe detoxification as required. Assist in the titration process for opioid individuals who use our services as required by service need to ensure the service user achieves an optimum dose of opioid substitution therapy (OST). This process is supported by good communication to other team members, including community pharmacists. Provide education, health promotion and screening for blood borne viruses (BBV). Provide Hepatitis A and or B vaccinations when indicated. Work within and consolidate integrated care and treatment pathways for individuals who are identified as being infected with BBVs (Hepatitis B, Hepatitis C and/or HIV). Undertake community alcohol detoxification either at the service or at the service users home. Support opioid detoxification in the community. Undertake phlebotomy, as required, ensuring adherence to identified pathways for blood sample labelling, collection, obtaining results and signposting. Use blood results to support the assessment of the current health status of individuals who use our services, the need for specialist medical referral and the suitability for community detoxification. Where indicated, refer for specialist inpatient services (including detoxification) and liaise with hospital and inpatient unit staff. Contribute to Change Grow Lives Naloxone Strategy by training staff and individuals who use our services in the use of naloxone. Ensuring naloxone is distributed to individuals who use our services at every opportunity. Offer wound care assessment and basic wound dressing to individuals who use our services. Organise signposting to other services as required and ensure clear pathways into other treatment arenas as appropriate. Have a demonstrable ability to manage emergency situations by being proficient in basic life support, defibrillation and managing a medical emergency e.g., anaphylaxis and overdose until the ambulance arrives. Understand and adhere to the legislation surrounding the use of patient group directions (PGDs). Utilise psychosocial intervention in both one-to-one and group settings to promote engagement in treatment services and ownership of recovery planning. Communication Develop therapeutic alliances with individuals who use our services and develop effective partnerships with them to support their individual recovery journeys, offering health promotion and harm reduction advice. Ensure that there is good communication of treatments offered, treatment progress and outcome of any investigations to other healthcare agencies, including GPs. Maintain a current knowledge of appropriate interventions. Ensure that all revalidation requirements are being met and communicated to line manager. Develop and provide health education, regarding problematic alcohol and drug use and alcohol and drug detoxification. Attend meetings appropriate to the development and maintenance of the role in agreement with and under the direction of the Lead/Cluster Lead Nurse. Delivering a Quality Service The post-holder will contribute to the service to ensure that people who use substances consistently receive high quality services that adhere to the best practice guidelines and to the highest professional standards. Ensure that procedures are constantly reviewed to promote best practice and evidenced based practice in accordance with UK Guidelines on Clinical Management, NICE guidance, and Change Grow Live clinical protocols and procedures. Leadership Provide clinical leadership within the service, including providing support, coaching, and teaching to staff working at the service around harm reduction including BBVS and health promotion. Dependent on service structure will provide line management/supervision to named health care assistants, student nurse placements and or volunteers working with the nursing team. Team Working To take part in assessing the suitability for transfer of clients to shared care services within the community. To work with others to ensure individuals who use our services have timely access to safe and effective prescribing for community detoxification from substances, across operational sites. Contribute to the overall performance of the service to ensure that contractual output targets are achieved. Working with colleagues in the service to design and deliver a community opioid and alcohol detoxification plan utilising a care co-ordinated approach to service user care. Risk Management Provide harm reduction advice to injecting individuals who use our services, providing information and equipment and ensure all equipment is disposed of safely. Undertake risk assessments and contribute to risk management plans for individuals who use our services engaged in the service. Utilise appropriate prevention techniques to reduce substance misuse related harm and to aid in the prevention of multiple physical harms caused by drugs and alcohol. Managing Information Record and input client data and information in order that the service operates within contractual, administrative, and financial requirements. Ensure that all data requirements in relation to individuals who use our services are met within defined deadlines. The above is an outline of the post holders duties and responsibilities. We have a new exciting job opportunity available for a Mental Health Practitioner within Sevenoaks Primary Care Network (PCN) in Kent.

The PCN consists of a group of 8 GP practices located in Sevenoaks, which includes Borough Green and Edenbridge, who work together in a local geographical area. The PCN is made up of a Multi-Disciplinary Team which includes GPs, Social Prescribers, Health & Wellbeing Coaches, Paramedics, Pharmacists and Physician Associates.

We are looking for an experienced and creative Mental Health Practitioner– this could be a Mental Health Nurse, Social Worker or Occupational Therapist - to work in the PCN as part of the Additional Roles Reimbursement Scheme (ARRS). The introduction of these roles is part of a wider transformation and expansion of community-based mental health services.

This is an exciting opportunity to improve the experience for individuals with mental health needs through offering early assessment and access to treatment and support to promote mental health and wellbeing in primary care, as well as getting involved in transformational work across the PCN.

The successful applicant will have access to regular supervision with a Senior Mental Health Practitioner, Professional Supervision, Group Peer Supervision and day to day support from members of the PCN MDT. The PCN also provides access to protected learning time.

The PCN is able to consider requests around both flexible and hybrid working.

The practitioner will be embedded as part of the PCN Multi-Disciplinary team and they can access and draw down on secondary mental health services and help provide a “bridge” between primary care and secondary mental health providers by delivering high quality mental health care in a Primary Care setting.

The ARRS mental health practitioner will be working with individuals aged 18+ with low-medium complex needs including risks that can be managed between the individual and GP . When the needs of the patient increase and the patient is unresponsive to the first line of treatment the mental health practitioner will refer the patient to Mental Health Together.

The role requires the practitioner to be a confident communicator and have the ambition and drive needed to work within an environment of change and innovation.

The practitioner will be expected to work autonomously & creatively in a busy and demanding role. Excellent assessment skills and knowledge of local resources are essential to this role, as it is the gate keeper and guardian for referral to secondary care services.

The Mental Health Practitioner will work closely with GPs to provide advice and support in caring for patients with mental health issues. There may also be opportunities to deliver training around mental health within the PCN.

You will be responsible for effectively managing and chairing the patient
Multidisciplinary meetings.

This work brings together a core group of Health & Social Care colleagues to discuss patient care, and plan treatment options and care outcomes.
You will need to have very good communication skills, be skilled and confident in chairing the meeting, and keeping to time.

You will be supported by an administrative assistant, to ensure the Multidisciplinary
meetings are set up and planned effectively.
With support from the admin assistant, you will manage the incoming referrals for
the MDT, ensuring all members required are invited, and have appropriate information prior to the MDT.

You will require strong IT skills to ensure all actions are uploaded to the correct
systems in a timely way.

Our West Kent Therapy Services are looking for motivated Occupational Therapists to work as part of our multidisciplinary services.

The West Kent Occupational Therapy rotation scheme will offer the opportunity to complete six months rotations, gaining experience across a range of community teams including:

• Community Hospitals
• Community Rehabilitation
• Neurorehabilitation
• Falls Prevention
• Urgent Therapy Service

You will gain experience across in-patient, clinic, group and domiciliary settings, and be able to build a wide portfolio of OT experience and skills working alongside our specialist teams.

These roles will suit those who are newly graduating, returning to practice or looking to move to a community role for a new challenge. We have a robust preceptorship programme for the first year in post.

Applicants will be required to have a full driving license and car, as being able to travel around the locality independently is essential to role

Your role will be to:

• To undertake all aspects of clinical duties as an autonomous practitioner with supervision from senior occupational therapists
• To be professionally and legally accountable for a designated clinical caseload
• To ensure comprehensive accurate electronic records are maintained for all patient interactions
• To contribute to the wider MDT

School Nurse – Near Sevenoaks
