Hours - 37.5 hrs per week working 5 days from 08.30 - 17.00.

Berkshire Community Dental Service is looking to recruit a permanent dental nurse.
The nurse will join an enthusiastic and skilled workforce with a great team spirit. The service has 7 clinics ranging from Thatcham to Langley and over 45 members of staff. The service treats patients with additional needs including children, people with learning and physical disabilities and the elderly.

You will be based at Berkshire Community Dental Clinic in Bracknell, but will also work at other clinics and in the domiciliary setting. It is therefore essential that you can drive, have a UK driving licence and a car for travel between clinics and hospitals.

There are excellent opportunities for gaining experience in treating patients under sedation and general anaesthetic. The service has hoists and a wheelchair tipper/bariatric bench. Professional development and training are encouraged. Annual leave is 5-6 weeks pro rata depending on experience.



The key duties are:
• You will work in a multi-surgery clinic, having support from other colleagues.
• You will assist in the provision of dental care for children with high levels of dental disease and children and adults with additional needs.
• You will have the opportunity to provide care in the domiciliary setting.
• You will assist in the provision of care for patients under inhalation sedation.
• You will assist on general aesthetic sessions.

The PCN consists of 3 practices serving 50,000 patients spanning over Bracknell/Crowthorne and Sandhurst. We are a friendly and innovative organisation, with a strong focus on education and personal development. We are delighted in being able to offer a Clinical Pharmacist position within the PCN to join our experienced and enthusiastic clinical team. The PCN strives to develop the MDT with support from the local training Hub, and with a training practice within the PCN, there is strong support and encouragement for clinical pharmacists to develop their skills and knowledge with both nationally recognised programmes such as the CPPE and ACP, and local courses such as in leadership, management of long-term conditions and minor illness. We also encourage the MDT to pursue special interests, with some of our pharmacists leading clinics in diabetic care and respiratory care. Job overview The post holder must be a registered pharmacist, who acts within their professional boundaries, supporting and working alongside a multidisciplinary team in general practice. The role will include patient facing services ranging from telephone consultations to face to face consultation. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy while addressing both public and social care needs of patients in the GP practices. The post holder will provide primary support to general practice staff with regards to prescription and medication queries, and will provide support in the prescription hub, providing expert advice on medicines, the repeat prescription system, manage acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing. The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services. The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. The post holder will be supported to develop their role to become a nonmedical prescriber. Main Duties and Responsibilities Patient facing longterm condition clinics: See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g. hypertension, asthma). Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e. medicines optimisation). Make appropriate recommendations to GPs for medicine improvement. Patient facing clinical medication review: Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacist, nurses and/or GP on prescribing and monitoring. Patient facing care home medication reviews: Undertake structured medication reviews with patients and produce recommendations for the senior clinical pharmacist, paramedics, nurses or GPs on prescribing and monitoring. Work with care home staff to improve safety of medicines ordering and administration. Patient facing domiciliary clinical medication review: Undertake structured medication reviews with patients and produce recommendations for nurses and GPs on prescribing and monitoring. Attend and refer patients to multidisciplinary case conferences. Management of common/minor/self-limiting ailments: Managing caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence. Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate. Patient facing medicines support: Provide patient facing clinics for those with questions, queries and concerns about their medicines in the practice. Management of medicines at discharge from hospital: To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Set up and manage systems to ensure continuity of medicines supply to highrisk groups of patients (e.g. those with medicine compliance aids or those in care homes). Signposting: Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate timeframe, such as pathology results, common/minor ailments, acute conditions, long term condition reviews etc. Repeat prescribing: Implement the practice repeat prescribing policy. Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review. Ensure patients have appropriate monitoring tests in place when required. Risk stratification: Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches Medicines quality improvement: Undertake clinical audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the practice team. Medicines safety: Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance. Implementation of local and national guidelines and formulary recommendations: Monitor practice prescribing against the local health economys RAG list and make recommendations to GPs for medicines that should be prescribed by hospital doctors (red drugs) or subject to shared care (amber drugs). Assist practices in seeing and maintaining a practice formulary that is hosted on the practices computer system. Auditing practices compliance against NICE technology assessment guidance. Education and Training: Provide education and training to primary healthcare team on therapeutics and medicines optimisation. KEY RESPONSIBILITIES The post holder will: 1. Have an appropriate experiential health care background such as: health care assistant, therapy assistant, emergency medical technician. 2. Work with The Digital Health Clinicians supporting high level remote monitoring for patients with short- and long-term conditions. Escalate to The Digital Health Clinicians when markers for deterioration are evident on submission of abnormal observations by patients via the Docobo system. Thereby, ensuring patients on the caseload who alert have clinical contact urgently. This ensures care is delivered in the community setting and aids prevention of unnecessary hospital admission. 3. Work with partners in nursing and residential care to ensure optimum health outcomes for their residents. 4. Work on shared caseloads with Integrated Care System (ICS) by acting as an integral member of the community team with a shared vision for developing caseloads as required by Integrated Care System partners. 5. Possess experience working within the telemedicine field to support the delivery of clinical care remotely; working, under direction of the Digital Health Team Leader and the Digital Health Clinical colleagues when required. 6. Deliver health promotion advice under supervision from clinical colleagues via the Digital Health model, e.g. chronic disease management self-help signposting. 7. Work in alignment with Primary Care, for established practice populations, toward a shared vision of seamless management for patients with short- and long-term conditions within the Primary Care and the Community setting. 8. Work with ICS partners in Primary and Social Care to further develop the Digital Health model for cohorts of patients managed within local populations. 9. Maintain accountability for practice to the Digital Health Team Leader following all EBPC Policies and Procedures. 10. Maintain legible, accurate and up to date records that are dated and signed in accordance with EBPC Policies for standards on record keeping. 11. Contribute to the onboarding of patients with short- and long-term conditions to the Docobo patient management platform. 12. Contribute to the discharging of patients with short- and long-term conditions from the Docobo patient management platform. 13. Work in a supervised capacity with patients, their families and carers managing referrals according to need and priority. 14. Escalate complex clinical data that has alerted on the Docobo patient management platform to the Digital Health Clinical colleagues. 15. Understand and support the achievement of EBPCs business plan objectives and performance targets. 16. Identify the potential for service developments, risks and deficits and inform the Digital Health Team Leader making recommendations based on vocational knowledge. 17. Be accountable for practice taking every reasonable opportunity to sustain and improve knowledge and competence. 18. Assist the Digital Health Team Leader to manage the effective use of resources within the Digital Health team and participate in regular review meetings with the Team Leader. 19. Have experience of Docobo patient management platform or similar. 20. Have experience with other clinical patient management systems: Adastra, EMIS, Connected Care PATIENTS CARE The post holder will: 1. Use telemedicine knowledge and skills to remotely monitor the physical and psychosocial needs of a defined client group. 2. Use telemedicine and healthcare knowledge to respond to alerts raised by the Docobo patient management platform. Urgently, escalating patient alerts to the Digital Health Clinicians to action. 3. Work with partners in nursing and residential care to improve the health outcomes of the residents and so prevent unnecessary hospital admissions or extended in-patient care episodes. 4. Assist the Digital Health Clinicians who are dealing with complex patient situations. Thereby aiding the establishment of a therapeutic relationship with individual patients using The Digital Health service. 5. Use telemedicine assessment tools/skills that will ensure an appropriate level of therapeutic supervision so that patients who present with complex needs are referred/escalated via the appropriate care pathway. 6. Conduct patient encounters with sensitivity and to act as patient advocate, maintaining patient confidentiality. 7. Deliver care that is patient centred, respecting diverse cultural backgrounds, and requirements. 8. Work in partnership with the patients to empower them to make informed choices about their healthcare and support choices. 9. Offer a supportive service to patients and their carers who are using the Digital Health service RESPONSIBILITIES FOR PEOPLE AND TRAINING 1. Be responsible for providing and maintaining a learning environment and maximise opportunities for education and development in the clinical area to enhance individual development and performance in the delivery of high standards of care. 2. Act as a Digital Health resource and assist in mentoring new Clinical Support Team Members in the Digital Health environment. Take part in the process of clinical supervision and participate in clinical supervision and mentorship of others. 3. Take responsibility for your own personal development; maintain competence, knowledge, and skills commensurate with role.

The must haves for you to be considered for this role: Recognised psychodynamic psychotherapy qualification and registered with UKCP as a psychotherapist or with the British Psychoanalytic Council (BPC); or ACAT accreditation as a CAT Practitioner or Psychotherapist and/or UKCP registered as a CAT Psychotherapist Experience of delivering brief, time limited psychological therapy interventions with complex mental health difficulties Comprehensive knowledge of Psychodynamic and/or CAT theory and practice and the ability to skilfully apply the model to formulate treatment plans, work therapeutically within a time limit and evaluate interventions If youre someone who shares our passion for excellence and care, join us and lets be outstanding together. Are you a Registered Mental Health Nurse with an interest in working in our CYPF Health Hub, CAMHS Common Point of Entry Service as a Mental Health Practitioner? Then we at Berkshire Healthcare NHS Foundation Trust (BHFT) have an exciting opportunity for you!

This fantastic opportunity will be responding to complex referrals to the CAMHS team, carrying out initial triage and telephone assessment of new children or young people including risk assessments and the formulation and recommendation for on-going care. You will review referrals coming into the service and undertake a structured telephone triage and develop a formulation to determine the mental health needs of the child or young person, assessing risk and either signposting to the appropriate CAMHS team or pathway.

You will also be providing advice and consultation to referrers, colleagues and families who may require advice about symptoms, risk, or the need for a referral. You will also be identifying those who need a more detailed face to face assessment within CPE and allocating a priority level.
1. Triage, assessment and formulation of young people presenting to Child and Adolescent mental Health services.
2. Consultation with professionals making referrals to the service
3. Undertaking Telephone assessments of young people referred to the service
4. Undertaking Face to face assessments of young people referred to the service
5. Work flexibly across multiple sites
6. Taking part in the review of clinical records
7. Reviewing of data to support the management of waiting lists
8. Working within a team of skilled CAMHS clinicians
9. Joining a service that provided front door access to a comprehensive CAMHS
