The Berkshire Early Intervention in Psychosis Service (EIP) is looking forapassionate and enthusiastic band6mental healthpractitionerto join their innovative and dynamic team. The EIP team is a trust wide service, with teams based at each locality. We are a specialist service working with individuals experiencing a first episode of psychosis or who are presenting with an at-risk mental state,between the ages of 18to 65. Weoperatea Monday to Friday 9am to 5pm service, although some flexibility is required. This advert isfor a rolecoveringSloughlocality, based atNew Horizons,Slough. You will have a clear passion for supporting people and their family in their recovery. You will work with our integrated team of psychiatrists, psychologists, psychological therapists, and STR workers to empower and support our clients to understand their experiences and return to college,universityor employment. We work with our clients for up to 3 years and the majority do get transferred back to their GP, therefore it is a rewarding team to work with.


• To act as care coordinator,in accordance withthe CareProgrammeApproach (CPA) policy under the supervision of the Clinical Lead.  The role is focused on working collaboratively with the client and their family/friends on their recovery, developing a relapse management plan. 
• To undertake comprehensive assessment of health and social needs together with the client and their family to implement a plan of care andprovision of NICE recommended interventions.   
• To work closely with other stakeholders to ensure the client has a comprehensive and holistic care plan to meet theirwell beingneeds. 
• Tomonitorand assess the physical health of service users in line with Trust policy and national guidelines. 
• Plan and provide treatment in collaboration with amulti disciplinaryteam and ensure clients are on the correct care pathway

This is a very rewarding role within Adult team at East Berkshire Psychological Medicine, based at Wexham Park Hospital. The role would suit an experienced practitioner from a mental health nursing background or other similar profession.

The post holder will be joining a well-established and supportive team working with adults requiring mental health input in Wexham Park Hospital. The team supports patients within the Emergency Department as well as medical wards across the hospital. The work is dynamic and there are many opportunities to develop close working relationships with specialist departments within Wexham Park Hospital as well as with other mental health services.

The post is within a supportive multidisciplinary team of mental health practitioners, including mental health nurses, psychologists, and a full-time consultant psychiatrist. The adult service is a small team and works alongside the older adult team within the East Berkshire Psychological Medicine Service at WPH. The overall service is an established Core24 provision, and is accredited with the Royal College of Psychiatrists.

We are looking for someone who is passionate about providing a holistic approach to care and displays a high level of compassion.

An informal visit to the service is encouraged.

The role involves shift working as the service runs 24 hours per day , 7 days per week and you will be expected to work various shifts. Staff work long days.

You will respond to referrals from the Emergency Department and wards, and complete full psychosocial assessment including identifying risk and formulate a safe discharge plan. In addition you will liaise with other departments and services as necessary to progress a safe discharge.

The inclusive age range is 16 years and above. The post holder will be responsible for the formulation, execution and evaluation of a care plan, including signposting to other services. You will ensure that each individual plan is developed in consultation/agreement with the patient and informed by the wider multidisciplinary team. You will operate within a multidisciplinary team and will be responsible for the delivery of care, risk management and evaluation of care, emphasising individual service user’s recovery. From time to time you will operate within the local Acute Trust as a senior practitioner, providing advice and specialist assessments to formulate appropriate plans of care, including signposting to other services. You will direct and supervise junior staff and / or students in the delivery of care to the post holders allocated service users or as required by the clinical team lead. You will work collaboratively with CRHTT regarding gatekeepers to hospital in-patient beds and Local Crisis Beds. You will be confident in taking positive risk in accordance with the Trust Policy around Risk Assessment and management.

2. Broad experience gained over a minimum of two years , acquired by caring for people with mental health needs in a range of community/inpatient settings

The post holder will work as an senior practitioner within the main Crisis Resolution Home Treatment Team Hub based in Slough. The overall objective is to provide a very high standard of care to patients under the care of the Crisis Resolution Home Treatment Team as an alternative to an acute mental health admission.

The post holder will provide high quality and comprehensive assessments, treatment and brief, evidence-based follow up service to patients referred from the community mental health teams; psych liaison service or inpatient wards

The post holder will be responsible for the formulation, execution and evaluation of a care plan, including signposting to other services. The post holder will ensure that each individual plan is developed in consultation/agreement with the service user and informed by the wider multidisciplinary team.

The post holder will operate within a multidisciplinary team and will be responsible for the delivery of care, risk management and evaluation of care, emphasising individual service user’s recovery.

The post holder will be confident in taking positive risk in accordance with the Trust Policy around Risk Assessment and management.

This is an exciting opportunity for the position of a Senior Mental Health Practitioner to join the Slough Community Mental Health Team (CMHT) at a time of new developments for the service.

Within the CMHT we have Senior Mental Health Practitioners who work as care coordinators in a longer-term way with people who live with severe and enduring mental illnesses. There are also a number of Senior Mental Health Practitioners who work in with smaller caseloads in a more intense, goal-orientated way for shorter periods. This is usually especially helpful for patients who require support to change their social circumstances.

The successful post-holder will work alongside dedicated clinicians from other disciplines: Psychologists, CBT Therapists, Psychiatrists, Occupational Therapists, Nurses and Social Workers. We work in a multi-disciplinary way, undertaking comprehensive BioPsychoSocial assessments, developing transdiagnostic treatment plans, and with a focus on reablement and positive risk taking.

Working as a joint-funded service (NHS and Local Authority) we undertake statutory roles on behalf of Slough Borough Council, such as Care Act 2014 assessments for eligibility, Safeguarding reviews etc.; as well as NHS care coordination and treatment delivery in the community.

As aSenior Mental HealthPractitioneryou will be a member of the Slough Community Mental Health Team (CMHT) based in New Horizons. Slough CMHT is a multi-disciplinary service.
