Opportunity available for ADHD Practitioner to work within a CAMHS Setting, Based in Bolton (Hybrid)

The team sits within a brand new Clinical Hub Setting with the role predominately responsible for assessments and associated follow ups from assessments and taking on patients who require ongoing titration from call backs and follow ups from Clinician leavers. As part of this role you will evaluate adults, and/or children, and young people for ADHD, considering potential co-occurring conditions to inform diagnostic decisions and apply clinical judgment and experience to make treatment decisions, utilising advanced skills and knowledge in managing ADHD in adults and/or children and young people. You will exercise independent professional responsibility for diagnostic decision-making within competence, whilst receiving monthly supervision from Team Leads and you will practice as a medical or non-medical prescriber (independent/supplementary prescribing) to develop and implement treatment and titration plans, ensuring physical observations are obtained and reviewed (height, weight, blood pressure, and pulse) ensuring treatment decisions are safe and in accordance with NICE guidelines and organisational policies. You will support the patient throughout their treatment journey, conducting regular follow ups for adults and/or children under your care until they have reached stability and optimisation and perform patient risk assessments and review safeguarding needs in conjunction with treatment strategies during the patient journey.

The hours for this role are Full Time with shifts being Monday to Friday
This role offers hybrid working with 4 days in the office (Bolton) and 1 day working from home

The hourly rate for this role is: £34.88 Per Hour

Manage and prioritise workload to ensure patient and service needs are met Manage clinical systems and appointment ledgers for both the Acute Visiting Service and Care Home Service Manage team rotas and service capacity Coordinate and manage administrative functions of both the Acute Visiting Service and Care Home Service Build relationships with PCN Member Practices Clinical and Administrative teams as well as Care Home team members Track and report service utilisation outlining allocations and booking completion across the Home Visiting Service Communicate effectively with patients and their families/carers Communicate with other members of the integrated care system if a patient needs to access other services Coordinate patients long term condition annual reviews Support administration of Personalised Care Support Plans (PCSP) for care home residents Attend service meetings and contribute to service delivery and growth Link with GPs and other clinicians in the PCN following visits to feedback patient outcomes where required Refer complex cases to the multidisciplinary team meeting and complete administration after the meeting and communicate this back to practices. Receive and collate information from hospital admissions and discharges and present this information to the team Work with the PCN management team on other projects from time to time, where capacity allows, such as collating data for quality targets across the PCN or health inequality projects

North West Ambulance Service NHS Trust provides 24 hour, 365 days a year accident and emergency services to those in need of emergency medical treatment and transport.

Our highly skilled staff provide life-saving care to patients in the community and take people to hospital or a place of care if needed.
We also provide non-emergency patient transport services for those patients who require non-emergency transport to and from hospital and who are unable to travel unaided because of their medical condition or clinical need.

Alongside the other emergency services, we also work to ensure the safety of the public and treatment of patients in the event of a major incident.

We also deliver the NHS 111 service in the North West. NHS 111 replaced NHS Direct in 2013. This service was introduced to make it easier for people to access local NHS healthcare services in England. It provides non-emergency medical help fast, and is available 24 hours a day, 365 days a year.
