We are looking for a number of Health Improvement Advisors to join our existing team and will contribute by directly delivering HHIS frontline adult health improvement interventions. You will be responsible for managing and monitoring client caseloads that support and encourage residents to stop smoking through a national standard treatment programme and/or receive an NHS Health Check or wellbeing check. The role involves face-to-face interventions which could be in one of our community clinics or on our Better Health Bus, as well as supporting and guiding residents via telephone or digital support. These roles will also support the delivery of work across some of our focused areas of work that address health inequalities such as pregnancy and mental health. The Health Improvement Advisor roles will work within HHIS to develop and deliver frontline services, working alongside national, regional and local policies and procedures, working as part of the wider HHIS team to ensure our services are promoted, delivered, and reported on and communicating with colleagues, partners and residents to work towards our agenda. Led by a HHIS Health Improvement Manager and line managed by a Health Improvement Coordinator, the HI Advisor roles will sit within a strongly established team, progressing objectives within their own portfolio of work that contribute to the wider HHIS aims and objectives.

The post holder is required to take responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care and management of a defined caseload, including liaison with other agencies and, where appropriate, the supervision, development and teaching of staff and/or students. The post holder will hold a case load of service users, ensuring the care plan is met at all times. To work as a member of the multi-disciplinary team. Job Responsibilities: The post holder will: Be responsible for the full assessment of care needs and risk assessment of service users presenting with a wide variety of clinical conditions following discharge from hospital. Following this the post holder will be responsible for the development, implementation and evaluation of programmes of care, without direct supervision (though clinical supervision is implemented as per policy to support you). To be named clinician, and act accordingly when applying care. To manage one self in a professional manner, responding to requests, enquiries etc in a timely fashion. To respond to any crisis that a service user may advise of, with the aim of promoting independent living in the community Work with the philosophy to support service users to remain in their own home. Administer and/ or supervise medications as required. To have a sound knowledge of medications and their desired and undesired effects, Be required to provide education, advise and guidance to service users and their carers and families. Provide a range of clinical interventions and treatments appropriate to the individuals needs. Participate fully in providing quality care in line with local and national guidelines. Act as a role model in providing a service for people with mental health problems. Participate in audit/research projects associated with the service. Be required to challenge practice that compromises or challenges high quality care to older people To advise Team Manager of any resource short falls. To partake in continuing professional development that will also benefit the development and quality of the service. Remain updated with local and national guidelines and policies. Partake in home visits with other professionals such as Consultant Psychiatrist, Specialty Doctors, Social Workers, community Matrons, Occupational Therapists etc Partake in clinical and management supervision

Undertake a range of duties as agreed by unit policies and protocols which involve direct patient care, under monitored supervision. Assist as instructed in the coordination of care delivery for a case load of patients for those with mental health problems including those with severe, enduring mental illness and Learning Disabilities. To work through and with the Directorate, psychiatric / learning disability multidisciplinary team, patients and other community agencies as instructed and guided. Contribute to the monitoring of the physical environment ensuring that it is safe and conducive to the therapeutic work of the unit/community setting, whilst being able to accurately monitor and feedback concerns the registered practitioner. Have informed knowledge and understanding in the psychological, social, physical and spiritual needs of all patients as guided as agreed in a monitored supervisory framework. Actively participate and assist in the care and rehabilitation of patients as per the plan of care determined and supervised by a registered practitioner. To report and feedback concerns within instructed duties, that facilitate timely comprehensive and contemporaneous written clinical records for sign off and oversight by a registered practitioner. Contribute towards the delivery of care using the recovery approach to provide therapeutic interventions as agreed as part of an ongoing and monitored supervisory framework. To support recovery outcomes as guided within a clear supervisory framework with a focus on care plans produced and evaluated by a Registered Practitioner. This role is fully flexible allowing you to work around any commitments, promoting a healthy work life balance.

Requirements:

• Previous or current UK experience in mental health support
• PMVA restraint training certification is essential.
