What to expect

You will be visiting clients in their own home in the Chester and Ellesmere Port area.

You will be meeting several clients each day, so you need to be a great communicator and even better listener, as our clients love to tell great stories.

You will be providing personal care and assisting with personal hygiene and dressing to get them ready for the day ahead.

We don’t need you to be a Michelin Starred chef, however basic cooking skills, such as boiling an egg, making sandwiches and using the microwave is essential.

Our clients have a range of ailments that they need support with, so our Support Workers need to be patient and have compassion.

We have a variety of shifts available, including morning shifts, full days and afternoons. We also have evening shifts 6.30pm – 11.15pm which attract an enhanced pay rate.

We provide support to our clients 7 days a week, so alternate weekend working is something we require.

Morgan Hunt is working with a national homelessness charity that delivers a wide range of support and housing services for young people and adults at risk of homelessness, including both emergency and longer-term housing and accommodation services.

An exciting opportunity has arisen to join their new Supported Lodgings service in Cheshire West and Chester. This service aims to provide long term placements for 16- and 17-year-old young people with care experience in the homes of vetted and trained Community Hosts. Working alongside the floating support and accommodation services that provide structured support packages, they enable young people to develop a range of skills as they move towards independent living.

This is a fantastic opportunity to truly make a difference to the lives of young people! Working remotely for the most part, you will manage your own diary and be given the support and encouragement to truly develop the role of Supported Lodgings Coordinator into its own.

The role

• First point of contact for young people and referral partners wanting to access the supported lodgings service.
• Lead on the assessment of needs and risk, predominantly focused on suitable risk management plans for the safeguarding, matching and placement of young people in the homes of hosts.
• Arrange all aspects of a young person's stay with a host, ensuring they feel welcomed and supported, considering the support needs of individuals.
• Ensure the health and safety of the environment for young people and hosts through regular checks and reporting.
• Provide regular support and planning sessions for young people,
• Work with young people, hosts and other agencies, to prepare them for independent living and access longer term, appropriate accommodation using support plans.
• You will be part of a team responsible for a 24 hour on call service - on call will be subject to staffing levels but currently this is approx. once every 3 months (a week at a time).
• Recruit, vet and train hosts both formally and informally.
• Be proactive in maintaining host' skills and motivation, including training and celebration events, and provide opportunities for them to shape service delivery.
• Carry out regular checks on host accommodation and assisting them to make necessary changes.
• Ensure that all hosts are made to feel safe, secure and valued.
• Proactively work in partnership with multiple agencies including statutory teams, to support the wellbeing and ongoing housing needs of young people, working in accordance with safeguarding, data protection and information sharing protocols.
• Lead on the active promotion and selling of the Supported Lodgings service to all potential hosts and referral partners, across the voluntary/statutory sectors and within the wider local community.

• Experience and an understanding of working with people experiencing homelessness, or people in crisis.

The Paramedic will provide care for the presenting patient from initial history taking and clinical assessment through to the diagnosis, treatment and evaluation of care. They will demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe care for all patients. They will work collaboratively with the healthcare team to meet the needs of the patients, supporting the delivery of policy and procedures. On a typical day, the Paramedic will be expected to carry out both face to face and telephone triage, there may also be a number of home visits depending on the demand. Mentorship and supervision will be provided by designated senior medical personnel. The level and type of supervision will be dependent on the post holders acquisition of skills and knowledge and determined by the organisations clinical governance arrangements. The practices hold regular education meetings and the Paramedic would be expected to attend and partake in these. Duties and Responsibilities Clinical Participate in all treatment and preventative healthcare services in the Practice as delegated and agreed by the supervising GP(s). Provide direct clinical care to patients using established clinical guidelines. Consult and refer patients to physicians, medical specialists and other health professionals as indicated. Interview patients, take medical histories, perform physical examinations, analyse, diagnose, and explain medical problems during surgery consultations and home visits. Attend regular educational meetings organised by the practice in order to update clinical knowledge, practice policy and guidelines and disseminate other useful information relevant to the provision of adequate healthcare for patients. Regularly reflect on own practice (and keep a record of learning encounters) in order to identify learning needs and encourage self-directed lifelong learning and continued professional development. Administration Send and receive written information on behalf of the Practice regarding matters of insurance, housing and other issues relating to the physical and social welfare of patients. Work to deliver with other members of the practice and Primary Healthcare Team, the objectives of the GMS contract, including the terms of the Quality and Outcomes Framework and Practice Based Commissioning. Attend regular multi-disciplinary meetings organised by the Practice in order to discuss the health and social needs of particular patients. Sous Chef
