FIXED TERM 12 MONTHS

Are you an experienced registered Mental Health nurse, Occupational Therapist or Social Worker looking to secure the next step in your career or to take on a new challenge within Older People’s Mental Health Services? If so, we are currently looking for an enthusiastic practitioner to join our fantastic team for 12 months secondment cover.

You will work with the clinical team and take responsibility for the assessment and implementation of intensive care and treatment in the home of people with mental health needs, as well as people with dementia. You’ll need at least two years of post-registration experience and specialist knowledge of the care of older people with mental health problems and mental health crises.

The role is based at Nicholson House, Maidenhead and your time in this role will be spent visiting clients in their homes in the local community so you will need a car to get to appointments. You will be required to come to the office occasionally for team meetings, training or for other service-related reasons as necessary, although currently we conduct most team meetings virtually this may change in future.

The service operates 7 days per week, so you will work shifts to cover the service hours of 9am-5pm, with some weekends (9am-3pm) and bank holidays as required.
• To manage and be responsible for the mental health care of an allocated caseload of patients.
• To undertake comprehensive initial assessments in a range of settings following acceptance of referral, taking into account the persons physical, psychological, social and spiritual needs.
• To develop and implement an agreed plan of care with the service user and where appropriate, include the views of families and carers.
• To provide a fast and flexible response to care needs, responding to referrals in a timely manner and providing intensive treatment (up to four times daily).
• To identify and select from a range of health, social and voluntary agencies which could assist and improve the care of individuals and their families.
• To initiate and carry out the agreed treatment plan
• To monitor and evaluate the agreed care plan, making adjustments as appropriate in collaboration with the wider team by reporting in review or CPA meetings.

Primary Purpose of RoleThe role of the rehabilitation assistant is integral to the service and vital in enabling the service to support those that wish to remain at home for end of life care, prevent hospital admission and enable residents to leave hospital earlier to continue rehabilitation and recovery at home. The role is supported by a team of therapists guiding and educating in the delivery of physiotherapy and occupation therapy programmes. Responsibilities include; To deliver safe and skilled person centred support to residents of RBWM in their own homes. This can include personal care, meal preparation, medications and rehabilitation programmes. To provide detailed feedback to the therapy team/service coordinators and duty officers to enable the rehabilitation plans to be reviewed and updated as the service user progresses back to independence, the service provides equipment to allow the team to manage this. To report any deterioration and or changes in condition to enable appropriate medical interventions to take place, this may include liaising with district nurses, GPs, social workers and other allied health professionals. To confidently use initiative and decision-making skills in order to keep the service users safe and free from harm in the community. To participate fully in weekly and monthly team meetings, supervision and appraisal process. To work with families and carers to explain, and support them to understand, the rehabilitation process. You'll assist with the annual budgeting process, review data (spend and forecasts) with Heads of and produce budget schedules for Committee and the Trust Board approval. You'll be detailed conscious and a forward planner who has excellent IT and planning skills to ensure the smooth operation of the team within budget Using your strong financial acumen you'll support others to manage budgets, find more for less solutions and lead a cost conscious culture, ensuring sustainability for the future. You'll work with Heads of to ensure that processes such as Gift Aid, VAT returns and other financial actions are managed appropriately. You'll ensure the Finance team and Company Secretary meet statutory responsibilities including but not limited to the annual accounts and Audit.
