Key Duties will include:

• Be the curriculum leader for PE and Health and Social Care, leading a committed team to securing great outcomes.

The Deputy Service Manager will provide support to the Service Manager in providing leadership and management for the delivery and provision of the NHS Health Improvement Practitioner Service across Bolton.



Health Improvement Practitioners (HIP) are a key part of the local health improvement workforce, tackling health inequalities and supporting the agenda to increase life expectancy of the Bolton population.



HIPs are based within GP surgeries across the Borough and work alongside primary care colleagues.



This post will be expected to support and provide a comprehensive reporting function, developing support systems to enhance service efficiencies and support performance management. The role

will facilitate a robust evaluation function for the service, subsequently monitoring the outcomes and produce analytical reports as required.

An essential part of this remit is to work strategically across the local health & social care economy, collaborating and negotiating with public health, primary care and social care. This collaboration will ensure the service is meeting exacting standards of service delivery and there will be successful progression of ad hoc projects and pathways.



The role will require an effective planner with exceptional organisational skills. The post holder will work autonomously across a range of projects and across organisational boundaries to support a dynamic and progressive HIP Service.

As part of the overall duties of the post this will involve generic HIP duties and tasks. This includes managing their own case load of patients, and providing a comprehensive behaviour change service for targeted groups of patients within primary care.

In summary, the Quality Improvement lead will provide leadership to teams across the trust to ensure delivery of a number of complex quality improvement change programs to drive continuous improvements in service delivery, using the GMMH adopted QI methodology. They will be an ambassador for Quality Improvement across the Trust and provide expertise, advice, education and support to ensure that improvements are delivered effectively and for the benefits of patients and staff.

The post holder will be part of the GMMH QI team based in the Nursing & Quality directorate, at Prestwich. They will be required to support staff and service users/carers trust wide.



The post has a key role in supporting the Trust to meet its vision of incorporating Quality Improvement into our culture, our way of working, and supporting organisational change.



The team collaborates with staff, service users/carers across the Trust to facilitate the use of QI methodology for improvement. The team is responsible for developing the Trust QI capability. They will have excellent organisational skills and the ability to deliver and have oversight of multiple projects.



The post holder needs to work autonomously and to support teams with improvement plans and initiatives. The post holder will have a pivotal role in ensuring safety is at the heart of everything we do. They will be expected to seek out good practice related to QI in the NHS and apply this in a consistent way across the Trust.



They will take ownership of data within work-streams ensuring that we measure, monitor and evaluate outputs to ensure that improvements are delivered effectively and for the benefits of service users/carers and staff.



The post holder will have leadership experience demonstrating the ability to line manage, coach and develop colleagues. It is essential that they have excellent communication skills and the ability to build relationships with staff, service users/carers and external stakeholders.

Essential requirements for the post include: A recognised Quality Improvement qualification. Prior experience of service re-design or quality improvement initiatives. Experience of leading change in complex systems. Knowledge and experience of the principles and application of improvement methodologies such as LEAN, collaborative models, PDSA and process mapping. The ability to analyse performance data, assess projects, identifying areas for collaborative working to improve outcomes. The Mental Health Liaison Service is an ageless service, working with patients of all ages. The post holder will be expected to have some experience working with a wide variety of patient demographics, including children and older adults.

The post holder will be required to work 37.5 hours a week including shifts, weekends, bank holidays, and nights. This service operates over 7 days a week 24 hours per dayand so there will be times when the post holder will need to frequently work out of hours to operate the service and meet patient needs.



The post holder will be a member of the Mental Health Liaison Service in GMMH – Bolton Urgent Care Services and will provide support and assistance to the professionally qualified staff in the department. The post holder will support patients placed in the urgent care centre, this is a waiting area for patient to wait for a mental health assessment to be completed by the Liaison Practitioners.

Responsibilities will include direct nursing care, cleaning of the waiting area, and delegated duties from the shift coordinator.
