We currently have a vacancy for a Community Mental Health Nurse (also known as a Care Coordinator) within our Heywood & Middleton CMHT.

The post will be based at Hanson Corner working within the Heywood & Middleton CMHT. However, this may be subject to change dependent on demand in our service.

You will need to be a registered and experienced RMN to apply for this post.

Key Responsibilities of the Post:
The Care Coordinator will provide a comprehensive mental health service to adults of working age who experience moderate to severe mental health problems within the community. The practitioner will also:
Work collaboratively across a range of disciplines to continuously promote a ‘needs based’ approach to all service users.
Promote an integrated approach to service delivery.
Develop clear relapse recognition / prevention plans and share with MDT, service users and carers where appropriate
Develop meaningful engagement with individuals who are often referred to as ‘difficult to engage’ and their families.



Role Summary:
The successful applicants will work effectively with adult service users with mental health needs across HMR CMHT, predominantly in the active care coordination function and will manage the care of a defined caseload of patients.

The post holders will be highly skilled and competent in conducting comprehensive holistic health and social needs assessments and risk assessments, developing outcome based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence based outcome measures with a focus on recovery and social inclusion.

The post holders will be required to provide a range of highly skilled interventions including group work and share those skills with other team members.



Service/Ward/Department Summary:
The CMHT work primarily with working age adults who have a diagnosis of a Serious Mental Illness and / or Complex Mental Health Difficulties. The care pathway includes a holistic assessment of need / risk, formulation and delivery of a care and treatment plan with a focus on recovery and supporting individuals to achieve and maintain their optimal level of functioning

About the role
Come and join the GV Health Rural Allied Health Team (RAHT), a team with a well renowned supportive workplace culture with an opportunity to work alongside experienced Occupational Therapists. You will work autonomously, undertake a varied and diverse workload and provide interventions in predominately home based settings, making a true difference to our client’s independence and well-being. An exciting opportunity has arisen in Hammersmith and Fulham CAMHS for a skilled and experienced Band 8a CBT Therapist to join the CCAMHS Team (Tier 2) in a full time, permanent contract 1.0 post.

We welcome applicants from a wide variety of professional backgrounds for example Psychologists (with BABCP accreditation), Social Worker (with BABCP accreditation), Nurse (with BABCP accreditation), etc.

The team is a large and growing multidisciplinary team, offering a range of targeted, early interventions to children and young people age 0-18 who are experiencing mild-moderate mental health difficulties. The team is now expanding its provision of CBT.

The service is committed to broadening accessibility and initiatives are being implemented to support this, including through the use of innovative digital tools as well as through embedding clinicians in community settings and developing relationships in community organisations such as youth groups, faith groups as well as in children’s centres, family hubs and GP settings. The post holder will join an established multi-disciplinary CAMHS which is responsible for providing Tier 2 mental health services within the borough. The post holder will work in Hammersmith and Fulham Community CAMHS Team with young people aged 0- 18 years. The clinic setting is based in central Hammersmith and there is some limited travel to other parts of the borough to attend community settings.

You will also offer line management and Clinical Supervision to junior CBT therapist and CBT trainees in the team and support some aspects of the developing wider CAMHS CBT pathway.

The post holder will provide mental health support and treatment for children and young people carrying out comprehensive assessments, including risk and the development of risk and safety management plans, and offering specialist interventions from CBT approach.

The current team consists of Child and Adolescent Psychotherapists, Systemic Family Psychotherapists, Clinical and Counselling Psychologists, Social Workers, CBT Therapist and Children’s Wellbeing Practitioners, providing multiple perspectives and allowing for rich MDT discussions and support.

Other aspects of the role include developing relationships with key partner agencies and community organisations working locally with children and young people, i.e. schools, GPs, social workers, community groups etc.

We currently have a vacancies for Community Mental Health Practitioners (also known as Care Coordinators) within our CMHTs.

The posts are currently predominantly based at Sudden Resource Centre working within the Rochdale West CMHT. However, this may be subject to change dependent on demand in our service.

You will need to be a registered and experienced RMN, Social Worker or OT to apply for this post.

The Care Coordinator will provide a comprehensive mental health service to adults of working age who experience moderate to severe mental health problems within the community. The practitioner will also:
Work collaboratively across a range of disciplines to continuously promote a ‘needs based’ approach to all service users.
Promote an integrated approach to service delivery.
Develop clear relapse recognition / prevention plans and share with MDT, service users and carers where appropriate
Develop meaningful engagement with individuals who are often referred to as ‘difficult to engage’ and their families.



The successful applicants will work effectively with adult service users with mental health needs across Rochdale CMHT, predominantly in the active care coordination function and will manage the care of a defined caseload of patients.

The post holders will be highly skilled and competent in conducting comprehensive holistic health and social needs assessments and risk assessments, developing outcome based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence based outcome measures with a focus on recovery and social inclusion.

The post holders will be required to provide a range of highly skilled interventions including group work and share those skills with other team members.

The CMHT work primarily with working age adults who have a diagnosis of a Serious Mental Illness and / or Complex Mental Health Difficulties. The care pathway includes a holistic assessment of need / risk, formulation and delivery of a care and treatment plan with a focus on recovery and supporting individuals to achieve and maintain their optimal level of functioning

To answer telephone calls in a polite and courteous manner dealing with any requests promptly and efficiently, returning call backs in timely manner.
To meet and greet any visitors and professionals to the Home

To support the manager in the positive marketing of the Home

To maintain accurate records both in e form and manual in relation to monies held in the home

To ensure the Manager is made aware of any potential problems or issues in a timely manner
To maintain clear and concise and legible records

Qualifications, Skills and Experience

Essential
