Epsom and St Helier hospitals are looking for a Locum Diabetes and Endocrinology Consultant to join the department for 1 year. The post holder will provide specialist care to patients, we are looking for an enthusiastic, proactive and patient care focused doctor.

Working within the multidisciplinary team, the post holder will play a key role, for both patients and health care professionals. The post will provide broad exposure in general medicine, diabetes and endocrinology.

This is a new Consultant post in Diabetes, Endocrinology and General Medicine. The post is full time including 1 PA for General Medicine on- call. A key role for the appointee will be to lead the provision of excellent care for people with diabetes in hospital by delivering daily in-patient diabetes reviews and working closely with the Diabetes Inpatient Specialist nurses and the Podiatry team.

Surrey Downs Health & Care

An exciting opportunity for a newly qualified or developing Band 5 therapist wanting to work in the specialist field of community neurorehabilitation.

We are looking for an enthusiastic individual to join our specialist neuro rehab multidisciplinary team working in Surrey Downs. You will be motivated to develop skills in the assessment and management of acquired complex communication and swallowing disorders and will support patients and their families with rehabilitation across community settings, through education, setting of patient-centered goals, and the use of evidence-based practice. You will have good access to the wider team, including regular supervision, and internal/external training opportunities, along with opportunities in the use of videofluoroscopy.

The post holder will be an experienced or newly qualified SLT with relevant qualifications and some expertise in adult-acquired swallowing and communication difficulties. This post will require knowledge, skills and judgment across a broad range of disorders and client groups including stroke, dementia, traumatic brain injury and progressive neurological conditions. The post holder will have their own caseload and would be responsible, with support as needed, for providing a service to adults in the community including domiciliary visits, nursing homes, community hospitals and End of Life Care. Collaborative working occurs within the multi-disciplinary team ensuring patient-driven goal planning and intervention. The post holder may also provide support within the stroke-supported discharge service. S/he will assess, plan and implement individual therapy programs and advise adults presenting with a wide range of communication and swallowing problems. Educating carers, family members, other professionals and supervising students are also an important part of the role.

S/he will participate in research, audit and development activities within the department. Working collaboratively with colleagues, as well as health care and social service personnel s/he will regularly be involved in departmental service development. An exciting opportunity has arisen to join the Renal service team at St Helier Hospital as an MDT Service Coordinator (non-clinical).

We are looking for an outstanding team player, an individual with a keen eye for detail and someone who can multi-task and independently prioritise their workload. The individual must be highly motivated and proactive.

The successful candidate will coordinate and run the Cardio - Renal - Diabetes MDT meetings, as well as undertake all associated administrative tasks, including tracking patients on their pathway.

The successful candidate will also coordinate and support the SWL ICB funded Supportive Care clinics and MDT for the Renal division.

· Ensure effective patient management plans for all patients referred into the Cardio-Renal-Diabetes MDT and SWL ICB funded Supportive Care service either via eRS, GP or consultants, for patients cases to be discussed with all relevant notes, imaging and treatment information available within 2 weeks of referral.

· To work as part of the Cardio – Renal – Diabetes multi-disciplinary team, acting as a liaison between consultants, nursing teams, community and primary care professionals, South West London ICB, admin teams, outpatient team, scheduling team, therapies team and theatres.

· To ensure a smooth patient pathway from MDT, to outpatient, inpatient and community follow ups after Consultant / Specialist Nurse and / or MDT Review and management plan has been completed.

Extracting data as needed from designated data sources to support NMTR and related high priority audits within the Trust.

Responsible for data submissions to NMTR relating to trauma patients

Validating data entered onto the NMTR database

Contributing to collation and analysis of data for designated clinical audits

Collates and completes data submissions for NMTR within agreed timelines, working with the trauma team and clinical audit team. This involves being responsible for the provision of timely, accurate and reliable data to NMTr. Identification (using NMTR inclusion criteria) of patients admitted with traumatic injuries and subsequent completion of data submissions via the NMTR. Liaising with the Trust Trauma Lead on a regular basis.

The Clinical audit team works across Epsom and St Helier aiming to embed a culture of assurance and continuous improvement.

Capturing the relevant information from the notes, required as part of the NMTR database. Submitting the appropriate data onto the NMTR database ensuring that the data is accurate and complete by using data from multiple electronic applications - e.g. Soliton (radiology system), patient manager (iPM) and Clinical Manager (iCM). Ensuring data is submitted in a timely manner in line with the national standards and those expected by the Trauma Unit. The trust doctor will be working as part of a team with Consultants, Registrars, a team of four other trust grade doctors, nursing staff and other allied healthcare providers. Duties include the care of patients in the outpatient setting, attending theatre, ward and accident and emergency care related to an on-call commitment and general middle grade duties.

Duties of the Post

• Assessment, care and treatment of patients presenting acutely to the specialty and referred to the specialty.

• Attending and running clinics and assessing patients and formulating treatment decisions seeking advice from the consultant as appropriate.

• Attending operating sessions with the consultant and assisting or performing operations as appropriate.

Clinical Responsibilities


· To provide a specialist level of clinical skill in paediatric occupational therapy

· Be responsible for a clinical caseload of children and young people attending Perseid special schools as identified by the service specification, managing the defined caseload independently and prioritising own workload to ensure effective service provision

· Use standardised and non-standardised assessment, interview and observation skills to interpret a range of factors relating to the client’s condition, determine needs and effectively set goals and plan intervention including implications for access to education, functional skills at home and the wider community

General Duties


· Be responsible for a clinical caseload of children and young people attending Perseid special schools as identified by the service specification, managing the defined caseload independently and prioritising own workload to ensure effective service provision

· Provide expert advice to clients/parents/carers and clinicians regarding provision of nutritional intervention based on clinical evidence. This will involve nutritional diagnosis (interpretation of biochemistry, anthropometrics, clinical condition and diet history), the development of treatment plans (calculation of nutritional requirements and client centred goals) and prescription of dietary treatment and ACBS products

· Provide appropriate dietetic service to children and young people attending Perseid schools who have complex special needs and/or enteral tube feeding

· Be involved in planning the development of the dietetics service in liaison with the school management team and the Service Manager for Children’s Specialist Nursing and Therapies

· Act as a resource to the SHC dietetics service and other agencies.

· Manage a specialist and highly complex clinical caseload as an autonomous practitioner

· Use and demonstrate highly specialist knowledge to inform sound clinical judgements/decision making for complex case management and advise others on the same

Service Manager for Children's Special Schools - 37.5 hrs per week
Band 8a, Fixed term - 4 months (until 31st Dec 2024)

Sutton Health and Care (SHC) is looking for an experienced, motivated and enthusiastic nurse or therapist to lead and manage the operational performance of the nursing and therapy services in Perseid and Greenmead special schools to ensure the delivery of high quality care to a range of children and young people with additional and complex needs.

This is a fixed term position until the end of the Autumn term initially.

The post holder will work in partnership with the schools' senior leadership teams to ensure that children and young people receive the care they require to support and enable them to work towards their education targets.

The successful candidate will be flexible, organised and creative with good IT skills and enjoy working collaboratively with other team members to support children and young people.

We are looking for a motivated and enthusiastic Band 6 Physiotherapist to work as part of the Sutton Health and Care Reablement Unit Team. The Reablemement unit provides an inpatient rehabilitation service for service users within South West London. Post holders will be working as part of an integrated multidisciplinary team including nurses, occupational therapists, physiotherapists, social workers, rehabilitation assistants, healthcare assistants and administrators, as well as the voluntary sector. The post holder will be based on the inpatient reablement unit, with some community visits providing rehabilitation over a 7 day period.
• To work as a key member of the multidisciplinary team in the assessment and management of a caseload of clients.
